Item type: Item , A 15-year study of hydatid cyst in children admitted to Tabriz Pediatric Medical Center(Tabriz University of Medical Sciences, Faculty of Medicine, 2020) Khodabandelou, Maryam; Abdoli-Oskouei, Shahram; Aslanabadi, SaeedThe aim of this study was to evaluate the cases of hydatid cysts in children admitted to Tabriz Pediatric Medical Center for 15 years. Materials and Methods: In this cross-sectional study, all children diagnosed with hydatid cyst who were referred to pediatric clinics in Tabriz during the last 15 years from 2003 to 2016 were studied. 120 patients were included in the study. Hydatid cysts in the lung were divided into right lung cysts, left lung cysts and bilateral cysts. In this study, the characteristics of hepatic hydatid cysts were examined and divided into left lobe cysts, right lobe cysts and bilateral cysts. Results: The mean age of the patients was 8.79±3.75 years and 59.2% of them were male. 52 patients (43.3%) lived in cities and 68 patients (56.7%) lived in villages. 77 patients (64.2%) had a positive relationship with livestock. The mean size of liver cyst in the studied patients was 55.45±24.19 mm with a range between 20 to 120 mm and the average size of lung cyst in the studied patients was 45.47±24.57 mm with a range between 15 to 130 mm.Item type: Item , 5 year study of CBC changes after scoliosis surgery(Tabriz University of Medical Sciences, Faculty of Medicine, 2022) Yousefzare, Siavash; Hosein Pouefeizi, Hojjat; Parish, MasoudScoliosis is one of the most common spinal deformities and due to the fact that the surgical incision to correct the scoliosis deformity is often large, these patients experience high bleeding and loss of high volume of the patient's blood volume requires blood transfusion. Increasing. Increased need for blood transfusions can lead to other postoperative problems and predispose these patients to postoperative complications; Therefore, the prediction and prevention of extensive blood transfusion is essential to improve the clinical outcome of patients. Therefore, in this study, we examine the changes in complete blood cell count in people who are candidates for scoliosis correction surgery over a period of 5 years. Methods: The sample of this study included 70 patients with surgical scoliosis who underwent scoliosis surgery in Shohada-Mahallati and Valiasr International Hospital in Tabriz in the period of 2015-2020. Gender, marital status, body mass index as well as surgery-related information including blood transfusion rate, duration of surgery, bleeding rate and duration of anesthesia were assessed. Also, red blood cell count, white blood cell count, blood platelet count, hemoglobin level, blood hematocrit level before surgery and up to 5 days after surgery were measured and evaluated. Results: The mean age of the sample was 24 18 18.43 years. In the sample, 65.7% were female and 34.3% were male. Examination of weight, height and body mass index showed that 58% of normal physical condition, 22% were obese and 20% were lean. The mean bleeding during the operation was 1213.89 ± 830 ml. In 85% of cases, blood transfusion was performed during surgery. Also, 30% needed blood transfusions after surgery. Intraoperatively infused blood was concentrated in 73% of erythrocytes and in 13% of both freshly frozen platelets and erythrocytes, and the mean total intraoperatively infused blood was 1098.6 ml. The results of linear regression showed that high weight (p = 0.007), low body mass index (p = 0.3) and high duration of surgery (p = 0.007) were significantly associated with high intraoperative transfused blood. The mean duration of hospitalization was 5.88 with a standard deviation of 2.1 days and the duration of hospitalization was positively and significantly related to the duration of surgery. Examination of the trend of changes in the variables of red blood cell count - platelet - hemoglobin and hematocrit showed that the day after surgery had a significant decrease compared to before surgery and this decrease continued until the third day after surgery. In all 4 variables mentioned, the decreasing trend changed after the third day and increased on the fourth day after the operation. The variable of white blood cells one day after surgery had a significant increase compared to before surgery, but from the second to the fifth day after surgery had a decreasing trend. The mean of variables 1 day after surgery was significantly different from before surgery (p <0.5).Item type: Item , 5-year demographic data of patients with chronic thromboembolic pulmonary hypertension(Tabriz University of Medical Sciences, Faculty of Medicine, 2022) Raeisnia, Khazar; Rashidi, Farid; Khezerloo Aghdam, NaserPulmonary hypertension due to obstruction in pulmonary artery because of chronic thromboembolism is classified in fourth group of all sorts of pulmonary hypertension. This disease causes heavy burden and affects the quality of life of patients. In addition, the prevalence of CTEPH is increasing and this can lead to an upsurge in mortalities. Main symptoms of CTEPH are dyspnea, activity intolerance, chest pain, edema and syncope. Materials and Methods: this is a retrospective cross sectional study of patients who referred to the pulmonary hypertension clinic of Imam Reza Hospital and were diagnosed with CTEPH. In order of collecting data a questionnaire containing all patients’ demographic information such as age, sex, history of deep vein embolism and thrombosis, clinical signs at the time of referral including dyspnea, chest pain, syncope and edema as well as patients’ hemodynamic findings was prepared. In basis of country protocol hospital emergencies use from five categories ESI(Emergency severity Index) in all of the country. The ideal triage criteria would be 100 percent accurate with no under triage or over triage ,however, this is not possible. We aimed to survey the ability criteria –directed protocol versus emergency severity index and modified rapid emergency medicine score in triage of trauma patients. Methods:This retrospective cross-sectional study was carried out on 120 patients admitted in Emergency department of Imam Reza hospital in 2020 where triaged by ESI and CDP .After study of patients we determine score of patients in basis of ISS and mREMS .If score of the patient more than 15 for ISS and more than 4-6 for mREMS ,the patient should be transmitted to CPR ,otherwise he or she under triaged. If score of patient lower than 15 for ISS and lower than4- 6 for mREMS the patient should be transmitted to trauma section ,otherwise he or she over triaged. American surgeon college advice maximum of under triage should not be more than 5 percent. In this study we calculated under triage 8.3percent and over triage 4.1 percent. Result :This study indicate ESI and ISS have direct connection in triage of patients. Also CDP and ISS have direct connection in triage of patients. ESI and mREMS have not direct connection with each other. CDP and mREMS have not direct connection and they are independent.Item type: Item , Abundance of Anxiety, Depression and Stress of vitiligo patients who referred to the dermatology clinic of Sina Hospital(Tabriz University of Medical Sciences, Faculty of Medicine, 2019) Sasan, Marefat; Farhang, Sara; Gareh Aghaji Zare, ArmaghanVitiligo is a relatively common acquired disorder of pigmentation characterized by the development of well-defined white macules on the skin that affects about 1% of the world's population. We sought to evaluate the diagnostic accuracy of NPBs as an emerging alternative to RT-PCR in the clinical identification of SARS-CoV-2. Our aim was to systematically synthesize evidence to justify the diagnostic value of NPBs as a suitable replacement for RT-PCR in the clinical identification of the novel coronavirus. Methods: We searched seven literature databases including EBSCOhost Web, Embase, ProQuest, PubMed/MEDLINE, Scopus, Web of Science, and WHO LitCOVID to identify original records concerned with development and application of NPBs in the clinical diagnosis of COVID-19 that were published from November 1, 2019 to April 30, 2022. We then calculated the indicators of diagnostic power including sensitivity, specificity and accuracy. As part of a joint clinical investigation, we collected a total of 72 saliva specimens 24 age-matched patients with RT-PCR-confirmed COVID-19 on days 1, 3 and 5 following their hospitalization, which were tested using the rapid antigen diagnostic kit for SARS-CoV-2. Results: We included 13 studies that reported original findings regarding the use of different NPBs in the clinical identification of COVID-19 via molecular identification of viral surface proteins and RNA in NPS/OPS. In total, 789 NPS/OPS samples gathered from 376 COVID-19+ and 413 COVID-19– participants were surveyed by the included studies using a total of 13 distinct NPBs. Compared to the reference test or RT-qPCR, NPBs yielded a mean sensitivity of 97.07%, a mean specificity of 94.43% and a mean accuracy of 96.91%. As for the clinical investigation, saliva-based test using rapid antigen kits conferred mean accuracy rates of 83.3%, 58.3% and 62.5% on days 1, 3 and 5, respectively.Item type: Item , The activity of different brain regions in fatigued and drowsy drivers: A systematic review based on EEG findings(Tabriz University of Medical Sciences, Faculty of Medicine, 2023) Norouzi, Ali; Ghojazadeh, Morteza; Farahbakhsh, MustafaDrowsiness and fatigue during driving is one of the major causes of traffic accidents, morbidity, and mortality in societies. AdeABC is a resistance-nodulation-cell division (RND)-type multidrug efflux pump conferring resistance to clinically essential antibiotics in A. baumannii, such as ciprofloxacin. This study aimed to target adeB gene with antisense peptide nucleic acid (PNA) and investigate its effect on resistance to antibiotics. NCBI database was used to design appropriate PNA to target adeB gene, by connecting PNA to mRNA, the translation of mRNA can be prevented. Three clinical isolates and A. baumannii ATCC 17978 were treated with the designed PNA by electroporation and competence procedure. Minimum Inhibitory concentration (MIC) of ciprofloxacin, colistin, and tetracycline were determined by microbroth dilution method. In addition, the expression level of adeB gene was measured by quantitative real-time PCR (qRT-PCR). Isolates used in this study had mutations in gyrA and parC genes corresponding to resistance to ciprofloxacin. MIC of resistance to ciprofloxacin after treatment with PNA was reduced from 32μg/ml to16μg/ml in A. baumannii ATCC 17978 isolate. Susceptibility level of tetracycline, in the 2 clinical isolates was decreased from 64μg/ml to 32μg/ml and in the other isolate was reduced from 128μg/ml to 64μg/ml. The expression level of adeB gene was decreased in A. baumannii ATCC 17978 (P > 0.01) but not in clinical isolate (P=0.107). Findings of the present study indicate overexpression of adeB efflux pump has extra effect on resistance to antibiotics in isolates with a defined mechanism of resistance. Antisense technology is a feasible technique to suppress the function of these genes, which may be further exploited to control multidrug-resistant isolates.Item type: Item , Adherence to diagnostic and treatment covid-19 guideline in outpatient care: viewpoints of medical Interns in Tabriz University of medical sciences(Tabriz University of Medical Sciences, Faculty of Medicine, 2022) Saeedifar, Sasan; Baradarane Binazir, MaryamDue to hospital beds limitation and widespread Covid19 pandemic, outpatient diagnostic and therapeutic procedures are one of the requirements of health centers related to Covid19. The clinical information of the patients including age, sex, cause of heart failure, underlying diseases, medications, laboratory findings, and echocardiography ECG findings as well as one-month follow-up were recorded and the recorded data were analyzed to find the relationship with early prognosis determination in these patients. Results: ECG findings such as ST Elevation with (P-Value: 0.04) and Pace Maker Rhythm with (Odds Ratio: 8.91) can predict readmission one month after hospital discharge in patients with acute heart failure. ECG findings were not significantly related to early mortality in patients with acute heart failure.Item type: Item , Admission renal function and electrolyte levels for estimation of total diuretic dose in acute heart failure patients(Tabriz University of Medical Sciences, Faculty of Medicine, 2024) Valizadeh Khoushehmehr, Mina; Chenaghlou, Maryam; Aslanabadi, NaserHeart failure is one of the major challenges in the healthcare system. Hospitalizations are a primary concern for these patients, imposing significant burdens on both the patients and the healthcare system. Identifying the factors that influence better management of these patients is crucial, as it can help reduce the overall disease burden. Typically, congestion is the primary issue in hospitalized heart failure patients, and the main treatment involves loop diuretics. Estimating the necessary diuretic dosage is important for better management of these patients. The aim of this study is to evaluate renal function and electrolyte levels at the time of admission to estimate the required diuretic dosage in patients hospitalized with acute heart failure. Methods and Materials: This study is a descriptive cross-sectional study with a prospective approach conducted in 2023. It included all patients diagnosed with acute heart failure who were admitted to Shahid Madani Hospital (affiliated with Tabriz University of Medical Sciences) through a complete census. All patient information, including age, gender, cause of heart failure, underlying conditions, medications used, laboratory findings, echocardiographic results, medications administered during hospitalization, and the total dose of furosemide administered, was recorded. The relationship between renal function and electrolyte levels at the time of admission with the total dose of furosemide prescribed during hospitalization was evaluated using regression methods, leveraging artificial intelligence and machine learning techniques. Results: In this study 250 patients with heart failure with mean age of 65/18±13.8 years and 65%of them were men. In these patients, the mean serum BUN level was 24/73±11/08 mg/dl and the mean total dose of furosemide was 540/88±223/46 mg. There was a significant relationship between BUN and the total dose of furosemide (P=0.033).
