Comparing mean left atrial pressure by invasive method and the mitral value surface area using 3D echocardiographic palnimetry

Abstract
Due to the limitations in mitral valve surface area measurement with plannimetry method, we decided to use a method for this measurement that has acceptable accuracy. The aim of the present study was to determine the role of the mean gradient and the mitral valve peak in assessing the severity of MS and to evaluate the relationship between the average left atrial pressure and the mitral valve surface area using the 3D method. Method: In this study, 135 patients with rheumatic mitral valve stenosis who were candidates for PTMC and referred to Shahid Madani Hospital in Tabriz during 1401-1402 were studied. After obtaining the demographic information of the patients and performing the 2D echocardiography study, the patients were subjected to trans esophageal echocardiography and then they were taken to the labrum to measure the hemodynamic pressures. Before performing PTMC, left atrial pressure and mean pulmonary artery pressure were directly recorded by the catheter. Results: Left atrial pressure was significantly higher in patients with mitral valve area ≤1 cm2 comparing patients with mitral valve area>1 cm. There was no significant difference in the mean pressure gradient between these two groups. Although in the subgroup analysis of left atrial pressures (greater than 15 and less than and equal to 15 mmHg), valve pressure gradients had significant differences and the mean valve gradient of 8.56±4.64 mmHg was associated with left atrial pressure above 15 mmHg.
