Evaluation of the outcomes of pancreaticojejunostomy anastomosis in pancreaticoduodenectomy surgery with and without the placement of pediatric double J stents

چکیده
Pancreatic cancer is one of the deadliest diseases with a poor prognosis. Pancreaticoduodenectomy (PD) is considered the gold standard treatment for resectable periampullary tumors. Postoperative pancreatic fistula (POPF) is one of the most common complications of this surgery, which can lead to infection, hemorrhage, and death. This study examines the outcomes of using a pediatric double-J stent in pancreaticojejunostomy (PJ) anastomosis during PD surgery. Materials and Methods: This retrospective descriptive study was conducted on 100 patients who underwent PD surgery at Imam Reza Hospital in Tabriz. Patients were divided into two groups: those with and those without the insertion of a pediatric double-J stent. The outcomes of these two groups were compared, including the incidence of POPF, hospital stay duration, postoperative complications, and surgeon satisfaction. Research Findings: Of the 100 patients, 49 were female and 51 were male. Thirty-three patients underwent classic PD, and 67 patients underwent PD with pylorus preservation. Overall, 14 patients developed POPF, with 10 cases in the stent group and 4 cases in the non-stent group. The results showed no significant difference in the incidence of POPF between the two groups (P-value=0.369). Additionally, other complications such as wound infection, biliary leakage, and respiratory and cardiac complications were observed in both groups, but there was no significant difference between them.
