Designing a structural reform model for the administration of public hospitals in Iran

Abstract

Abstract Introduction: Public hospitals are one of the largest providers of health services in Iran. The results of various studies conducted in Iran indicated that reforming the structure and management of these hospitals is the main way to improve their performance. Therefore, this study aims to design a model for reforming the structure of the administration of public hospitals in Iran. Methods: This study is a qualitative research with Mixed methods approach which was conducted in 2019. The present study was conducted in 4 general stages, including the first stage of gathering relevant global and national experiences, qualitative assessment of the current structure performance and determining appropriate approaches to reform the structure and finally validate the proposed model. Findings: The model presented reform of the structure of Iran public hospitals which combines the results of qualitative interviews with experts in the field of hospital management, including health system policy makers, hospital top managers, and conducting a systematic review in providing health care in the world, cathegorized in 10 dimentions of structural reforms in the areas of decision-making system, finance, education and research, payment system for providers, hospital information management, patient and family-centered hospital, human resource management, health prevention and promotion, reform and Improves administrative and support processes and promotes quality and patient safety Conclusion: The proposed model is a comprehensive and practical model and covered most of the topics that needed to modify the structure. Therefore, it is expected that if the developed model will be supported by the senior management of the Iran Ministry of Health and Medical Education, it can be implemented in the current structure of public hospitals in Iran and will have the good results.
