Predictive Value of Airway Preoperative Examination in Recognition of Difficult Intubation in Diabetic Patients undergoing Gynecologic Surgery

Abstract
The aim of the present study was to evaluate the predictive value of preoperative airway evaluation tests in diabetic patients undergoing gynecologic surgery. Materials and Methods: In this cross-sectional study, 75 female patients with type I or II diabetes, over 18 years of age, ASA class II-III and scheduled for elective gynecologic surgery with tracheal intubation were studied. Airway evaluation was performed by tests of "Malampatti", "Rule 3-3-2", "Upper lip biting", "prayer sign" and "Palm print". The association of each test with the difficulty of intubation by Cormack-Lehan grading was recorded. Results: Study patients had 26.7% difficult Malampatti, 21.3% un-normal 3-3-2 rule, 13.3% difficult ULBT, 16% positive Prayer sign and 30.7% difficult Palm print. Frequency of difficult intubation was 10.7%, and the most complications of intubation was hypertension with frequency of 32%. The highest sensitivity were related to Palm print and Malampatti with sensitivity of 100 and 87.5, respectively. The highest sensitivity of pair airway management tests was related to Malampatti with Prayer sign and Malampatti with Palm print and were both 100%.
