Abstract Introduction: Accurate, comprehensive, well-structured, well-managed data is required to control and prevent oral diseases, clinical audit, and quality of patient care fulfillment we need. Therefore, these data must be managed correctly. Objectives: This research was performed to present a pattern of dental information management system for dental faculty. Methods and Materials: In the current cross-sectional (descriptive) Survey, information management system in dental faculties of Tabriz, Tehran, Shahid Beheshti, Isfahan and Shiraz in 2013 has been studied. Data were collected through observing and questioning by checklist and questionnaire (self-constructer) tools. The validity and reliability of the tools were assessed by content validity and test-retest approach, respectively. Finally, according to the existing situation, a pattern was proposed for dental faculty. This pattern’s validity was investigated by a panel of experts using the Delphi technique. Data were analyzed by utilizing descriptive statistics as well as SPSS16 software. Findings: Results of this research demonstrate that maximum and minimum dental data elements related to dental faculties of Tehran (173 data) and Tabriz (160 data) respectively. Filing method in most of the faculties is done in different ways and only in Shahid Beheshti is performed Source Oriented Medical Record (SOMR) method. In most of this faculties, mainaintain duration of dental recoeds depends on the storage space limitation. Only Tabriz uses the International Classification of Diseases to Dentistry and Stomatology (ICD-DA) book. None of the faculties has a defined procedure in order to access level. Therefore, a five-axis model was proposed (data elements, mechanisms and processes of data collection, storage and retrieval, B classification systems and dissemination of information). After applying Delphi technique, the model was approved with a 73-100% agreement. Conclusion: The existence of a high quality and dynamic outpatient information management system is critical. Therefore, the processes and rules on data collection, storage, retrieval, classification and distribution in dental information management systems require more attention.
