Evaluation and Strategy Provision to Promote the Indicators of Integrated Geriatric Care Program in Yazd County: A Participatory Action Research

Abstract
Conclusion: Our results showed that the indicators of the Integrated Geriatric Care Program before the implementation of the action plan were weak and unfavorable in the dimension of reach, and the evaluation of the remaining dimensions of this program was reported to be fairly favorable. Finally, it seems that the implementation of the action plan in the present study was successful in improving the program indicators, and in general, the evaluation of the program after the intervention reached the favorable level. The results of this study help identify the challenges of the program and provide the opportunity for researchers, health policymakers, and providers of older adults services to improve care programs and remove existing barriers. The present study showed that Participatory Action Research can be used to empower and provide innovative recommendations for improving the health of older people in the future.
