Evaluation of the sensitivity and specificity of trauma patients referred to the emergency department of Imam Reza (AS) Hospital according to TRISS score
Abstract
One of the main causes of death currently is trauma, triage and prediction in trauma patients is very valuable, for this purpose, several scoring systems have been created that help in predicting the outcome of trauma patients, so the purpose of this study Evaluating the correlation between NISS and ISS and comparing it with TRISS in predicting the possible mortality rate of trauma patients referred to Imam Reza Hospital (A.S.), on the basis of which the severity of injuries can be checked and the probability of mortality can be calculated, as well as The degree of application of these ratings in the trauma treatment system was evaluated and possible problems were evaluated
Method: In this study, which is a cross-sectional descriptive study. Multiple trauma patients who referred to the emergency department of Imam Reza Hospital (AS) were included in the study, which was 494 based on study records and calculated with the formula of sensitivity and specificity, of which 495 patients were included in the study. First, ISS (Injury Severity Scoring System) was calculated for the patients and NISS (New Injury Scoring System) was also calculated at the same time, then TRISS was calculated as the gold standard predictor.
Results: 495 patients were included in this study. Based on this, the average age of the patients was 36±20.19 years, the median age of the patients was 36 years, and the interquartile range was 20-50 years. In examining the gender distribution of patients, 308 patients (62/2) were male and 187 patients (37/7) were female. In examining the outcome of the studied patients, 54.1% were discharged and 3.8% died. According to the investigation through the ROC diagram, considering the level under the diagram equal to 0.995, the sensitivity and specificity of ISS at the cut-off point 23 is 55.34% and 100%, respectively, and in the NISS study, drawing the ROC diagram with the level under the diagram is 0.994 , the sensitivity and specificity of this criterion at the cut-off point of 22.5 for predicting death were 76.32% and 100%, respectively, and also the sensitivity and specificity of the TRISS criterion in drawing the ROC chart with the area under the chart equal to 0.920 at the cut-off point 28.55 for predicting death is 94.12% and 99.37%, respectively.
