Prevalence of Delirium in Elderly With Coronary Artery Bypass Graft Surgery After Hip Replacement In General Anesthesia

Abstract
Patients who have undergone coronary artery bypass graft surgery may be prone to delirium and also patients who have undergone heart surgery should undergo general anesthesia if they need surgery such as pelvic joint replacement. Anesthesia drugs can also affect the incidence of delirium after surgery (the effect has not been determined). We decided to investigate the prevalence of delirium in the elderly with a history of coronary artery bypass graft surgery, a candidate for hip replacement after general anesthesia. The results of this study can be used in care and preventive planning for the elderly at high risk for delirium. Materials and Methods: This descriptive cross-sectional study was performed in 2019-2021 with the participation of 60 patients after hip replacement surgery with a history of CABG in Imam Reza and Shohada hospitals in Tabriz. All patients had a history of heart disease and the Nichmann criterion (delirium measurement) was performed for all of them on the first, second, and third days after surgery. Finally, the data were analyzed by Chi-square at a significance level of less than 0.05. Results: Delirium was observed in 31 people (51.66%) in this study; The prevalence of delirium in this study was 51.66%; The delirium score based on the Nichman criterion was 25.19±2.14, which indicated mild delirium in the participants. Based on the results of the logistic regression model, it was found that the age above 65 years is 4.69 times, the duration of surgery more than 150 minutes is 9.2882 times, the duration of anesthesia is more than 180 minutes is 14.37 times, hemoglobin After surgery, less than 10, according to Andara 3.99 times, bleeding during surgery more than 1500 cc. 19.96 times, chronic pain before surgery 25.47 times, CRP before surgery more than 0.6 times 8.96 times and CRP after surgery more than 0.9 times 14.59 times the increase risk of delirium.
