The aim of the above study is to investigate the difference between the Health-Related Quality of Life and Sexual Performance in people related to colorectal cancer in the group with and without colostomy bag. Methods: The current thesis is a descriptive-Cross-sectional study that was conducted in 2021-2022 in the city of Tabriz at Shahidmadani, Ostadalinasab, Shahriar and Valiasr hospitals. The statistical population of this research consists of people with colon and rectal cancer who have bag and also those without bag. The number of samples of this research is 256 people who were included in the study as available sampling. In order to collect data, valid and reliable questionnaires of international erectile function index for men, female sexual function index for female samples, health-related quality of life for both sexes and demographic information were used. The filled questionnaires were entered into IBM SPSS Statistics version 24, and then the data were analyzed using descriptive statistics and analytical and general linear modeling and multiple regression were used to control the effect of confounders. Results: The mean and standard deviation of the total HRQOL in patients with a bag is 43.03±17.08 and for patients without a bag was 51.38±15.57, the total score of the FSFI in patients with a bag was 7.21±6.40 and for patients without a bag was 14.67±9.10, the total score of the IIEF was 26.17±15.30 for patients with a bag, and for patients without a bag, it was 29.05±17.14. There was a significant difference in health-related quality of life between patients with and without bags in the total group and women (Respectively: P=0.002 and P<0.001). There was a significant difference in the FSFI in some dimensions in women with and without bags (P<0.005). In the IIEF, there was no significant difference between men with and without a bag (P>0.005). Conclusion: According to the findings of the research, it was determined that the HRQOL and sexual performance is disturbed in this group of patients, so it is suggested that decisions and strategies be adopted to improve the HRQOL and sexual performance, and from these people the necessary support should be implemented, especially it is necessary to pay special attention to those dimensions that need more support, which are the general health, physical performance and pain in HRQOL, the dimensions of lubrication, orgasm and pain in FSFI, and finally regarding IIEF, the dimensions of orgasm and sexual desire are among the dimensions that require more support.
