Comparison of the prevalence of infection after extraction with and without suture on posterior mandibular tooth socket in patients with controlled diabetes

Abstract
Introduction: Management of diabetic patients often presents a challenging and difficult situation for dentists. According to the available evidence of the negative effect of diabetes on wound healing, the aim of the present study was to investigate the prevalence of infection with and without suture in the mandibular extracted posterior tooth in people with controlled diabetes. Material and Methods: In this case/control study, 42 patients referred to the Faculty of Dentistry in Tabriz, Department of Oral, Maxillofacial Surgery, who met the conditions for entering the study and had bilateral extraction posterior teeth, were included in the study. HbAlc test, panoramic graph of the desired areas was received. With the injection of local anesthesia of the mandibular nerve block type, he was subjected to a minimally invasive surgery without cutting to remove two posterior teeth, each of which was on one side of the lower jaw (mandible), one week apart from each other. At the end, a figure of eight suture was applied by 0-3 monofilament nylon material on only one of the two extracted teeth of the patients. During the study period, on the third and seventh days after surgery, patients were examined for infection, pain, warmth, redness, inflammation, pus discharge, swelling, and trismus. The data was analyzed by SPSS25 software and statistical tests of REPETED MEASURE, PAIRED SAMPLE T TEST and Chi-Square. Conclusion There was no significant difference between the two groups with sutures and without sutures, in all three investigated times, the amount of pain, the frequency of pus, the amount of swelling based on anatomical criteria, and the amount of trismus. The frequency of warmth, redness and inflammation was higher in the group without sutures. There was a significant difference in the frequency of swelling intensity on the third and seventh days between the two groups. In both groups, a significant decrease in pain, inflammation and swelling was observed on the seventh day. This study confirmed the absence of a significant difference in the prevalence of infection after extraction of teeth with and without sutures.
