Protein, glucose, LDH, CRP, ESR and differential cell count of pleural effusion and blood in predicting complications of pleural catheter in patients with malignant pleural effusion

Abstract
The significant effect of malignant pleural effusion on the survival and quality of life of affected patients, predicting the complications of various treatments such as pleural catheter to alleviate the symptoms of the disease, in order to introduce suitable effective and low-complication treatment methods in these patients is of particular importance. is. The aim of the present study was to investigate the parameters of protein, glucose, LDH, CRP, ESR and cell count and cell differentiation of pleural fluid and blood in predicting pleural catheter complications in patients with malignant pleural effusion. Materials and Methods: This study was conducted on 40 patients with malignant pleural effusion referred to the lung department, lung ICU, or clinic of Imam Reza Medical Education Center in Tabriz during 2018-2019. At the time of pleural catheter insertion, the first sample of the patient's pleural fluid was taken for analysis in terms of protein, lactate dehydrogenase, glucose, cell count and cell differentiation. At the same time as the pleural fluid sample was taken, the patient's blood sample was also taken to check protein, lactate dehydrogenase, glucose, ESR, and CPR. Then, the follow-up of the patients was done to check the clinical conditions of the insurance and examination of the catheter site. Finally, the data were subjected to statistical analysis. Results: In this study, the average LDH of the pleural fluid was 546 (40-6836) and the blood sample was 482 (190-1857), the average pleural fluid glucose was 100.4 ± 50.1 and the blood sample was 135.1 ± 39.1. The average protein of pleural fluid was 3.9 ± 1.4 and blood sample was 6.2 ± 1.1. According to the pleural fluid sample, the average WBC (50-30,000) was 675 and the average lymphocyte percentage was 57 ± 16.6. According to the blood sample, the average ESR was 47 ± 21.4 and the average CRP was 33.3 ± 17. In 50% of the studied patients, pleural catheter was associated with complications, the most common of which was chest pain. The mortality rate was 77.5%. Although there was no significant relationship between any of the measured laboratory parameters and the complications of the catheter with the mortality of the patients (all cases P>0.05), however, with a decrease in the level of glucose in the pleural fluid, the chance of chest pain complications due to the pleural catheter increased. found (P<0.05).
