The effect of normal saline injection in medial nerve epineurium during carpal tunnel syndrome surgery

Abstract
There are three different surgical techniques for median nerve release in patients candidates for carpal tunnel syndrome surgery, including open or classical surgery, limited surgery and endoscopic surgery. Paraesthesia and drowsiness in the limited surgical method of carpal tunnel syndrome is always about 2-4% more than the classical method, but the return to work in this method occurs earlier than the classical method. The hypothesis is that it is possible to reduce postoperative complications by increasing the diameter of the nerve during surgery; Therefore, the aim of this study was to evaluate the effect of normal saline injection on median nerve epineurium during carpal tunnel syndrome release surgery. Materials and Methods: This study was a randomized clinical trial with a control group and one-sided blind during 1399 (beginning to end of the year) in Imam Reza Hospital (Tabriz University of Medical Sciences) with the participation of 120 patients with severe symptoms of tunnel syndrome Carp (based on the result of electrodiagnosis) was performed. Patients were randomly divided into two groups of intervention and control (60 people in each group). The surgical method was limited to all patients in the form of incision and for the intervention group after the release of the median nerve, one cc of normal saline was injected into the nerve and for the control group only the release was performed without injection of normal saline. Pain intensity, disease severity were measured based on electrodiagnosis findings, Boston questionnaire scores and duration of return to work between the two groups at regular intervals and compared between the two groups. Results: Measurement of pain intensity at regular intervals indicated that pain intensity was non-significant in the intervention group than the control group. Comparison of the results within the group also showed that the rate of pain reduction in the intervention group was better than the control group; However, this rate is insignificant and was not statistically significant; the severity of symptoms in both groups after surgery was significantly improved at all times; it should be noted that at the end of the study it was found that in 3 patients in the intervention and Mild symptoms were seen in 5 patients in the control group. Return time, hand strength, and functional status were statistically significant among study participants between the two groups; Scar pain was seen in 3 people in each group; Paraesthesia and drowsiness were seen in 4 patients in the control group and in 2 patients in the intervention group.
