Effect of Neoadjuvant Chemoradiotherapy on Downstaging of Locally Advanced Cancer of Cardia and patient’s survival

Abstract
Neoadjuvant chemoradiotherapy has recently been accepted as part of the treatment protocol in patients with localized advanced gastric tumors. The aim of this study was to evaluate the response to this diet and tolerance to it. Methods: A total of 49 patients with locally advanced gastric tumors were selected and after performing the required tests, they underwent neoadjuvant therapy with FC regimen and radiotherapy. Complications such as anemia, thrombocytopenia, neutropenia, fever and neutropenia, diarrhea, nausea and vomiting were reported if they occurred during chemoradiotherapy. Patients were re-evaluated 4-6 weeks after the end of chemoradiotherapy and the stage of the disease was determined and patients who were diagnosed as resectable by the surgeon were ready for surgery. After surgery, patients were followed up and patient survival was assessed. Results: Tumor stage in patients before and after neoadjuvant chemoradiotherapy was significantly reduced (P=0.043). The effect of neoadjuvant chemoradiotherapy on reducing the disease stage was 71.4%. It was also found that the rate of surgery of patients before neoadjuvant chemoradiotherapy was 24.5% (12 patients), which increased to 40.8% after chemoradiotherapy (P=0.008). The mean survival time of patients was 11.24±4.31 months, which decreased with increasing disease stage (P = 0.001).
