Application of national early warning score in prognosis of stroke patients in the emergency department
Abstract
Considering all the cases and that regarding the prediction of the acute stroke patient in the emergency room and the decision on the treatment method along with the routine scoring of stroke patients, there is a need for accurate and good criteria or evaluation systems. Only one study was performed on patients with acute stroke, so the aim of this study was to evaluate NEWS in patients with acute stroke and its relationship with treatment and type of stroke and patient outcome.
Methods: In this study, which is a prospective cross-sectional study. Patients who entered the emergency room of Imam Reza Hospital with acute stroke (symptoms of neurological deficiency that started acutely) were included in the study. Patients who died before filling out the checklist or the patient's condition due to emergency interventions did not allow the calculation and filling out of the checklist, patients who entered the emergency room intubated were excluded from the study. Then for each patient with acute stroke the checklist was filled and the patient's NEWS score was calculated. Data collection method: Using a data collection form designed by the researcher.
Results: In this study, 333 patients were included in the study. The average age (62.98-62; 12% CI) is 64.55 14 14.55 years, with a minimum age of 24 years and a maximum of 91 years. The average age is 63 years. Age between 75% - 25% equals 76-34 years. In evaluating the diagnostic value of NEWS regarding stroke type, based on ROC diagram, the area under the diagram is equal to 0.526 and considering the high intensity of NEWS above 7, the sensitivity (22.47% - 1.75%; CI 95%) 33.8% and specificity (97.15% - 91.81%; CI 95%) is 94.95%
