Endometriosis Painful Symptoms and its Relationship with Quality of Life in Women referring to Educational Centers Of Tabriz University Of Medical Sciences, 2019

Abstract
Abstract Background: Endometriosis is one of the most common and costly diseases of women of childbearing age. It is one of the leading gynecological causes of hospitalization in industrialized countries and its prevalence is increasing. Endometriosis threatens life quality, especially in pain, physical and mental function, and is associated with significant defects such as pain and decreased social function. This study aimed to determine the relationship between endometriosis's painful symptoms and women's quality of life with this disease. Method: This descriptive-analytical cross-sectional study was performed on 169 women with endometriosis. Random sampling was performed from two educational-medical centers (Al-Zahra and Taleghani) in Tabriz, Iran. Socio-demographic and obstetrics questionnaire, Pelvic Pain and Gynecologic Symptoms Questionnaire (ENDO PAIN- 4D) in patients with endometriosis, and Endometriosis Health Profile-30 (EHP-30) questionnaire were used to collect data. Pearson correlation test was used to determine the relationship between painful symptoms of endometriosis and quality of life in bivariate analysis, and the Adjusted General Linear Model was used in multivariate analysis with control of socio-demographic and obstetrics characteristics. Results: The mean (standard deviation) of the overall ENDOPAIN 4D score was 66.0 (61.0), from the score range of 0 to 320. The mean (standard deviation) of the overall EHP 30 score was 35.9 (22.4) from the achievable score range of 0 to 100. In terms of quality of life, the highest mean score was related to the emotional domain [42.0 (20.3)], and the lowest mean score was related to the pain domain [7.2 (4.5)]. According to the Pearson correlation test, there was a significant positive correlation between the overall score of EHP 30 and the overall score of ENDOPAIN 4D (r = 0.55 and p > 0.001). Based on the general linear model and by controlling confounding variables, with increasing the score of painful menstrual symptoms, the quality of life score increased (B = 0.2; CI: 95%: 0.1 to 0.2), explaining that on the EHP 30scale, as the score increases, the quality of life deteriorates. Conclusion: This study showed that the more severe the painful symptoms of endometriosis, the lower the quality of life, so it is necessary to reduce the painful symptoms of endometriosis by adopting appropriate treatment approaches such as lifestyle changes and self-care which can improve the quality of life of these patients.
