Three layer technique for reconstruction of buccal cortex in immediate implantaion of anterior maxillary tooth

چکیده
Introduction To achieve a successful result in immediate implantation technique, the integrity of socket walls, particularly the buccal wall is crucial. Defects in buccal wall during immediate implantation can prevent the new bone to regenerate to an acceptable amount. This study aims to regenerate the buccal cortex in the maxillary anterior single-teeth immediate implantation with 3-layer technique. Methods and materials In this case series study, 8 patients were studied. Once the tooth with severe buccal bones defect was extracted, primary bone and gingival height was measured. Dio implant dental fixture system was immediately implanted inside the socket and the bone substitute was placed on the buccal side of dental fixture. At the next step, an absorbable collagen membrane was fixed over the defect. Finally, free connective tissue graft from the palatal area was harvested and placed over the collagen membrane. After 6-month of follow-up, buccal surface of bone and gingival tissue of dental implant in the area of mesiobuccal, midbuccal and distobuccal was measured. The defect if presented was determined based on adjacent healthy teeth. Findings 8 patients including 3 females and 5 males with the mean age of 37.49 ± 9.9 were included in this study. The increase in bone in mesiobuccal, midbuccal and distobuccal was 27.41%, 60.47% and 61.46%, respectively. Gingival regeneration in distobuccal, buccal and mesiobuccal was 75.24%, 74.21% and 37.26%, respectively. After the follow-up, the success rate of the technique was 100% and none of the implants failed. Conclusion Three-layer technique can regenerate the buccal cortex in maxillary anterior single-teeth immediate implantation with high success rate.
