Cost- effectiveness evaluation of early detection of HIV in pregnant womenin suburban Kermanshah

Abstract
Abstract Introduction and Purpose: AIDS is one of the biggest health – social problems in many societies in the 21 st century. The disease is expanding because of low awareness of society and the lack of proper treatment. The present study aims to determine the cost – effectiveness of HIV / early diagnosis test in pregnant women in the affected areas of Kermanshah. It is a study of the kind of study of economic assessment studies, which involves the systematic assessment of costs and the benefits of screening, and is one of the methods of measuring cost – effectiveness analysis. Materials and methods: This study is a cross – sectional study of the kind of study of an economic assessment study comparing two types of strategy - making strategies to detect positive HIV cases in pregnant mothers in order to quickly identify them and carry out preventive and timely preventive measures to prevent infant mortality. the research community showed 200 people who were charged with HIV screening at public and private health bases in Kermanshah city. the cost of services provided by experts and reliable studies were obtained by reviewing the literature and instructions from the ministry of health. Also, the sensitivity of diagnostic tests and utility tests were obtained using validated studies. The parameters of this study included the prevalence of HIV in pregnant mothers, sensitivity and test property that were the true positive sensitivity (TP)and the real negative characteristic, the cost for treatment of pregnant mothers, expectant mothers with HIV, the hope of living in ordinary people and the hope of living in children with the main parameters of this . The analysis was calculated using the decision – tree method and the parameter related to the effect of the interventions was calculated by the evidence available in other studies to ensure the accuracy and robustness of the model and the results obtained by a one – way analysis analysis on several important parameters to determine the probable uncertainty in these parameters on the additive cost index Findings: The increase in cost – effectiveness index indicates that in a test strategy or testing a year longer is achieved with less cost of 19495515.68027 - rial, i.e. one year of life on the test strategy with this much lower cost. Conclusions: The sensitivity of diagnostic tests from the systematic review studies was extracted from the analysis of the sensitivity of all of the evaluated parameters, that at different levels of the variation of these parameters still the strategy of carrying out pregnant mothers toward strategy screening strategy was more productive or dominant, thus the study of the parameter for the desired parameter is very high. Cost – effectiveness survey showed that the strategy for testing is less expensive than the strategy of failing to test lower costs and more effect, so it is chosen as a better strategy.
