The impact of triamterene/hydrochlorthiazide plus furosemide versus hydrochlorthiazide plus furosemide in patients with refractory heart failure : A randomized clinical trial.
Abstract
Introduction: Cardiovascular diseases are one of the top 5 causes of death in the world today. So far, despite numerous clinical trials, no cure has been found for acute heart failure. The only way to treat heart failure is to control it with medications. One of the common medication regimens used to overcome refractory edema is combination of a loop diuretic with a thiazide diuretic and a loop diuretic with potassium-sparing group. Triamterene synergistically potentiates the effectiveness of thiazide diuretics by potassium-sparing mechanism. However, no research has been done on comparing the effect of treatment with furosemide plus hydrochlorothiazide and furosemide plus triamterene/ hydrochlorothiazide.
Objectives: In this study, the main purpose was to evaluate and compare the effectiveness of furosemide-hydrochlorothiazide treatment regimen with furosemide-triamterene/ hydrochlorothiazide in patients with refractory edema with congestive heart failure.
Methods: This study was performed as a randomized trial in Shahid Madani Heart Hospital of Tabriz University of Medical Sciences. 45 patients with the condition were randomly assigned into two groups of 22 and 23 patient s (furosemide-hydrochlorothiazide and furosemide-triamterene/ hydrochlorothiazide). Changes in the levels of elements such as sodium and potassium were assessed during admission. Weight changes, urinary fluid levels, urea and creatinine were also evaluated and assessed during admission.
Results: The results showed that there was not a significant difference in the measurement parameters between the groups of patients treated with furosemide plus hydrochlorothiazide and the group of patients treated with furosemide plus hydrochlorothiazide.
Conclusion: In this study, it can be concluded that furosemide- triamterene/ hydrochlorothiazide treatment regimen showed no difference in terms of efficacy compared with furosemide- hydrochlorothiazide regimen.
