Plasma Levels of sodium in patients with Pneumonia

Abstract
The aim of this study is toinvestigation of plasma levels of sodium in patients with pneumonia. Materials and Methods:This study was a descriptive study and was performed on all patients with definitive diagnosis of pneumonia (based on clinical and laboratory criteria) who were admitted to Imam Reza and Sina educational and medical centers of Tabriz University of Medical Sciences from the beginning of 1998 to the beginning of 1999 and their files were available (169 patients).Sampling was done census. The data collection methods by reviewing the records of patients.The questionnaire included demographic characteristics, sodium, urea and plasma glucose levels of all patients at the time of admission to hospital, length of hospital stay, need for intubation due to pneumonia and lobe involved in the lung. Serum sodium levels between 125 and 130 were classified as moderate hyponatremia and below 125 as severe hyponatremia. Results:In this study, the median age (25th and 75th percentages) of the subjects was 50.0 (36.5-60.0) years. Based on laboratory findings, the mean (standard deviation) of sodium levels at the time of arrival and the last day of treatment in the subjects of this study were 138.16 (±4.7) and 140.54 (±4.3) meq / l, respectively. Also, the mean (SD) of their potassium level was 4.07 (±0.6) mmol / l.median (RQS) of urea and creatinine levels in the subjects of this study were 28.5 (20.3-48.0) and 1.0 (0.9-1.2) mg / dl, respectively. Also, the median (RQS) of their glucose level was 119.5 (108.3-182.0) mmol / dl. In this study, 2 cases (1.2%) had moderate hyponatremia and 1 case (0.6%) had severe hyponatremia. The onset of symptoms was less than 6 weeks in 59 cases (34.9%) and over 6 weeks in 110 cases (65.1%).The most common clinical symptoms in this study were cough, shortness of breath and fever with 70 cases (44.0%), 64 cases (40.3%) and 43 cases (27.0%), respectively. Also, the most common type of lung involvement in these individuals was turbidity of both lung lobes with 28 cases (16.6%). The highest results of smear test and PCR in the subjects of this study were TB negative and influenza negative with 18 cases (10.7%) and 12 cases (7.1%), respectively.The median (RQS) of their hospital stay was 7.0 (5.0-9.0) days. Maintenance serum in the subjects of this study in 15 cases (9.4%) Half saline, in 47 cases (29.6%) Normal saline, in 19 cases (11.9%) 1/3 + 2/3 and in 3 cases (1.8%) dw5%. In this study, 10 cases (6.3%) of the subjects underwent intubation. Also, 4 cases (2.4%) of the subjects died. In this study, serum sodium level had no statistically significant relationship with mortality and maintenance serum in the subjects (P <0.05).
