Abstract
Abstract: Introduction & Objectives: Since the advent of fire, burn treatment has been concerned with the human being. By passing the time many scientific and practical studies in the field of wound care has been carried out, such that it can be cited, types of dressings. Generally burn wound care dressing to modern and traditional methods is done. Currently a variety of modern dressings as biologic and synthetic are available. It is thought that modern dressings facilitate healing of burns and they do not have limitation of traditional dressing. The other hands, studies show that high cost of care for burn patients. In this context analysis and cost effectives of nursing cares can help to approach from several actions to choose the appropriate, that yet be cost effective. The present study investigated the cost effectiveness of biological, synthetic and traditional dressing in nursing care of burn wounds. Materials & Methods: This research is clinical trial on 90 children with 2nd and 3rd degree burn wound, in Tabriz Sina hospital, burn center during 9 months, and in 2088-2009 was done. Population studied were children admitted to the and among them those who have a unit profile were studied. After informing parents on research subject and obtain their written consent, samples were selected randomly in two experimental and control groups. In experimental group A, 30 patients treated by synthetic dressing, in the experimental group B 30 patients with biological dressing and also in the control group 30 patients were treated with traditional dressing. Hospital stay, time to heal, severity of pain, infection, direct & indirect costs were evaluated using research tools including: checklist and questionnaire. Analysis of data with SPSS and descriptive and inferential data's test with chi –square & Kruskall –Wallis were performed. Results: The results showed that among three methods of dressing in terms of start to healing, pain severity, direct and indirect costs there are significant difference (p<0.0001) which means healing has happened sooner and hospitalization and severity of pain was lesser, also reasonable direct and indirect costs is spent. Related to the infection rates between 3 methods was not significant. Conclusion: In this study we hypothesized that there is statistical difference between the three methods of dressing that cost effectiveness was confirmed. Means that synthetic dressing in compare with other types of dressing was more cost effective. This shows that lack of utilization of modern products and methods applications, just because of high cost is not reasonable, and in this field it's necessary to discuss the cost effectiveness of new methods should be more attention given to it.
