The Effect of Home –based Cardiac Rehabilitation Program on Self –Efficacy and General Health of Patients referred to Tabriz Shahid Madani Cardiac Rehabilitation Center, 2009.

Abstract
Abstract: Background and Objective: The prevalence of heart disease is increasing worldwide as well as in our country so that 46 percent of cases of mortality has won. Community health nurses act in the third level of prevention with the aim of limit disability and complications and provide rehabilitation for individuals with impaired function. Cardiac rehabilitation is an interdisciplinary activity with the aim to facilitate and improve the physical, psychological and emotional state and empower patients in order to achieve higher levels of health, through exercise, relaxation and health education both in hospital and community. This study aimed to determine the effect of continuing cardiac rehabilitation programs at home on self –efficacy and general health in patients referred to Tabriz Shahid Madani Rehabilitation Center. Methods: A quasi –experimental repeated measurement study was conducted on 80 patients with inclusion criteria who had been referred to Shahid Madani Rehabilitation Center in case and control group with simple random sampling. Data was collected using the general Self Efficacy Scale (GSES) and Goldberg General Health Questionnaire (GHQ-28) and analyzed using Minitab software and repeated measurement analysis model. Results: This study shows that 75 percent (60 people) of participants were male and 25 percent (20 patients) were female. Patients' age range was 57.41±1.01 (Mean ± SE). Results showed statistical significant difference in self –efficacy (p=0.003) and general health (p=0.000) between two groups and in different measurements. Conclusion: The study results showed significant improvement in self –efficacy and general health of patients after conducting cardiac rehabilitation intervention. So referring patients who suffer from heart disease for recovering their ability and making them ready for independent life recommended. Community health nurse as a major team member has an important role in educating patients and their families in all stages of rehabilitation (hospital, outpatient and home).
