Continuation and Reasons for Discontinuation of Depot Medroxyprogesterone Acetate (DMPA) Injectable Contraceptive among Women referring to Ardebil Health Centers, 2005.

Abstract
Abstract: Introduction: Inadequate contraceptive is common among sexually active female adolescents, resulting in a high incidence of unwanted pregnancy. Depot medroxyprogesterone acetate (DMPA) a long –lasting injectable progesterone is highly effective contraceptive with a failure rate less than 1% of that is used in many part of the world including Iran and used by 6.26% of urban women in Ardebil. There are various variable such as the duration and client consistency in using a special contraceptive methods as well as some individual determination which play an important role in optimal using of different contraceptive methods. The purpose of this study is to determine continuation of DMPA among women referring to Ardebil health centers. Materials and Methods: This retrospective descriptive study was designed with a sample population; 396 women that have been injected their first dose of DMPA between March 21.2002 and December 21.2003 at the urban health center in Ardebil. Sampling method was satisfied randomizations. Data collecting tool was a questionnaire, statistical analysis method were carried out through, frequency, standard deviation, analysis of variance (ANOVA) with subsequent post hoc multiple comparison (LSD technique) and correlation. Results: The total sample amounted to 396 women. The mean age of the study population was 32±7.8 years, most of them were housewives 376 (95%), and the women 31% were illiterate. 6,9,12 and 24 month cumulative continuation rate of DMPA was recorded as 44.5%, 26.8%, 18.2% and 2.3% respectively. The most common compliant was amenorrhea and observed in 69.75 of women and most common reason for discontinuation was amenorrhea and observed in 54.5% of women after one year. DMPA continuation rate was significantly related to the level of education, level of husbands' education, job of husband, family income, number of Gravida, number of children and type of previous delivery (p<0.05). Conclusion: All personnel involved in family planning projects are aware that to attract new users, they must keep previous users. Counseling should emphasize the possibility of side –effect and inform women that most side –effects are transient and follow up visits should be scheduled for 6 months after a prescription is written. For decreasing economic hurts on society with increasing the DMPA continuation rate.
