The Effect of Education Based on a Collaborative Care Model on the Lifestyle Patients with Type II Diabetes and their Family Members.
Abstract
Abstract: Background and Aim: There is a strong relationship between life style and prevalence of type II diabetes. Regarding the increasing number of patients with type II diabetes and that it is a family disease, the aim of this study was to determine the effect of education based on Collaborative Care Model on life style of patients with diabetes type II and their family.
Methods: This research is a single – blind randomized clinical trial carried out in Educational Center of Diabetes of Tabriz in 1392. The sample consisted of 49 diabetic patients along with 40 first –degree family members (one individual from each family) that were selected based on objective and were randomly divided into two groups of experimental and control. The questionnaire of demographic characteristics and life style were filled before the intervention in both groups and collaborative care model was performed in experimental group for 3 months onstage of motivation, preparation, engagement one month after the intervention with pre -intervention data using SPSS 19 and Chi square.
Results: After the intervention, the patients of experimental group and family members has better performance in the variable glucometer. Also, the family members of patients of experimental group measured their blood sugar more (70%). On physical dimension, the patients and their family member of experimental group reported better performance in variables of walking as exercise in last 3 months and physical activity. On nutrition dimension, altogether, the patients and their members in experimental group had better performance in comparison with control and observed nutritional suggestions well. On stress dimension, also, the patients and family members in experimental group had better performance.
Conclusion: Performing collaborative care model on life style patients with diabetes type II and their family member has positive effect, so applying this model by nurses can improve life style patients and their families.
