Evaluation of electrophysiological Pattern and Prognosis of Neuropathy and Myopathy in Critical Patients Admited to ICU over 5 Year (2013-2018) in Tabriz Imam Reza Hospital

The aim of present study are ditermination of incidence of CIN, CIM, combined CIN/CIM and some acute neuropathies, electrophysiological pattern and prognosis among ICU admitted patients who had four limbs muscles weakness. Method and Materials: in this cross-sectional analytical study, critically ill ICU admitted patients who had four limbs as well as respiratory muscles weakness such as diaphragm muscle, which extubation from ventilator was not possible, were referred to electrodiagnostic center of Imam Reza university hospital by pulmonologist during a five year period (2014-2019). Complete nerve conduction studies of sensory and motor and phrenic nerves as well as needle electromyography of limbs and diaphragm muscles was performed and results were analyzed by statistical analysis. Results: The results of 50 patients showed that the most prevalent neuromuscular disorder was CIN with incidence of 19 (%38). The most underlying cause for ICU admission was respiratory failure, thromboemboli and then central causes. In average, among all patients, mean tiial nerve action potential was 1.13±1.18 mv, mean sural nerve action potential was 2.97±5.22 (µv) and phrenic nerve was 0.24±0.2 mv, which were not statistically significant different between groups. Duration of hospitalization in CIM/CIN patients was more longer than the others compared to GBS, CIM and CIN groups (p=0.029, 0.019, 0.035, respectivly). The most mortality was among CIN patients in 10 (%43 of the deceased) and then in CIN/CIM patients in 9(38%). The most denervation potentials was present in CIN/CIM patients. The most recovery and good NCS and the least mortality were among GBS patients and its variants.
