Determination of the diagnostic value of FIB4, Fibroindex, APRI, FORNS, LOK indices in diagnosis of portal hypertension in cirrhotic patients

Abstract
The timely diagnosis of portal hypertension in prognosis of cirrhotic patients is important Moreover, determination of the noninvasive diagnostic method is important in diagnosis and follow-up of cirrhotic patients with portal hypertension. On the other hand there are limited studies in this regard. So, the present study was designed to assess the prognostic value of serum indexes in diagnosis of portal hypertension in cirrhotic patients. Method: One hundred two cirrhotic patients were selected according to inclusion/exclusion criteria. Cirrhosis and also the child pough degree were determined by gastroenterologist. Portal hypertension was diagnosed by radiologist using color Doppler method. Moreover, fasting blood sample was drawn and different serum indices including liver enzymes, Albumin, platelet, INR, globulin and total cholesterol were determined. Finally the following indices were calculated: FIB4, Fibroindex, APRI, FORNS, LOK. Results: The mean age of participants was 59.54±12.69 years. About 52.94% patients were men. The mean liver enzymes level was higher in patients with portal hypertension. From the etiological point of view, in 52% of patients the etiology of cirrhosis was viral hepatitis. There was significant differences between two groups regarding the studies indices (p<0.05). According to the results of ROC curve, FORNS index was better predictive of portal hypertension in cirrhotic patients. Moreover, the maximum AUC was for FORNS index. According to the results of diagnostic tests, FORNS index had the adequate sensitivity and specificity in diagnosis of portal hypertension with the cut-off value of 8.51.
