Evaluation of Medical Care’s Status in ST-elevation Myocardial Infarction Patients in Isfahan Shahid Chamran Hospital in 2018

Abstract
Introduction: Immediate access to medical services and care for patients with ST-elevation myocardial infarction is completely necessary. Aim: The aim of present study was evaluation of medical care’s status in STEMI patients in Isfahan Shahid Chamran hospital. Materials and Methods: This study was performed on 200 patients with STEMI who has been hospitalized to and discharged from Shahid Chamran Hospital in Isfahan in 2018. Demographic data, patient discharge status, underlying disease and clinical information including prescribed medications were collected from admission to discharge period. The information was obtained from clinical guidelines and credible scientific resources related to the management and treatment of patients with STEMI and the latest version of Medscape software. Results: All patients received the first line standard medications including antiplatelets, anticoagulants and statins. Ninthy six percent of patients received proton pump inhibitors, 88% beta-blockers, and 77% angiotensin converting enzyme inhibitors and angiotensin receptor blockers at the time of admission and hospitalization. The pattern of drug administration was complete in accordance with the clinical guidelines. In 5% of cases, some dosage errors were detected. Drug interactions were observed in the all cases, with 10.03% of serious and 89.96% of monitor closely interactions. Almost 77% of the interactions were pharmacodynamics, 20% were pharmacokinetic and 3% were both types. In 1.5% of cases, complications due to drug interactions were reported. Conclusion: Despite the observed interactions and side effects in co-administration of some medications, their administration is in complete accordance with clinical guidelines. However, Failure to record a number of medical side effects in patients' records and medication dosing errors were observed that need for effective supervision on hospital protocols and advanced education on scientific and legal aspects of filling out a patient health profile.
