Estimating the direct cost(s) of injuries resulting from road traffic accident in Guilan Province 2012

Abstract
Background and aim: Most mortalities and morbidities in middle and low-income countries are due to traffic accidents which result in different outcomes including financial burden. This study aimed at determining the direct costs of traffic accidents in 2012 in Guilan province0 Methods: This study was a research in health system which included victims of traffic accidents admitted to medical centers in Guilan. ICD-10 was used as diagnostic criteria. Data were extracted from HIS system in Poursina Hospital in Rasht (capital of Guilan). Hospital costs were determined in age groups based on vehicle and final outcome. Effective factors on accidents were also considered. Costs were estimated based on Ministry of Health cost standards for all services. Results: Calculated Hospital cost was 104,908,338,912 Rails (8556690, 75 USD) with mean of 13679533,04 Rails (1115,7 USD) and CI:13140000,68-14218000,4 Rails. Total costs were 82,718,468,560 Rails for men (6747020,27 USD) with mean of 13846412, 55 Rails (1129,39 USD) and CI:13224000,91-14467000,19 Rails. Total costs for women were obtained 22,189,870,352 Rails (1809940/48 USD) with mean of 13091368, 94 Rails (1067, 81 USD) and CI:12022000,39-14160000,49 Rails. The highest cost belonged to 20-29 years age group and vehicle (car accident with motorcyclist). Para clinic, clinical and pharmaceutical costs and bed cost for patientfamily were 7,016,552,880 Rails (572312, 63 USD), 4,957,299,950 Rails (404347, 46 USD), 9,496,893,995 Rails (774624, 3 USD) and 1,354,951,800 Rails (110518, 09 USD). Cost for each death was 19,663,889 Rails (1603, 9 USD), respectively. Upper extremities had the most injuries. Marital status, age and type of vehicle were influencing factors on accidents0 Conclusion: Results indicated that high costs of traffic accidents constitute a major part of health system total costs. Developing a single trauma system and trauma care programs are recommended0
