Evaluating pattern of using Cardiac inotropes (+) (Dopamine, Dobutamine, Milrinone) in ICU ward of Tabriz Shahid Madani Hospital
Abstract
Background : The cardiac Inotropes are used to increase cardiac output and contractility via Intravenous Route and inculuding Dopamine, dobutamine and milrinon.
Aim : The aim of this study was to evaluate the use of cardiace inotropes inculuding Dopamine, dobutamine and milrinon in the setting of open Heart surgery based on AHA/ACC guidline.
Method : This is a prospective- descriptive study which was conduccted on patients undergoing open heart surgery in Tabriz Shahid Madani heart center in 2019, during a six months.
Patients, demographic data, drug and medical history and clinical data such as Blood pressure, Laboratory Results, Indication, dose, Rate of infusion, adverse drug Reactions were recorded.
Data were analyzed in SPSS and then compared with AHA/ACC guidelines.
Results : During 6-months study, a total number of 589 patients were entered to the study. Among them 156 patients Received Inotropes including 89 (57.1%) patients in ICU2 and 67 (42.9%) in ICU1.
Off 156 patients, 84(53.8%) were male and 72(46.2%) were female.Among 156 patients, 62 patients (39.5%) Received Norepinephrine. Regarding the Indication, 85.5% of Dopamine, 66% of Dobutamine, 90.3% of milrinon uses were based on the guidelines.
Morcovere, dopamine and dobutamin and milrinon were Rationally used in 66.6%, 49%, 58% of case based on the guidelines.
The treatment outcome were achived in 87.7% of dopamine and 78% of dobutamine and 71% of milrinon.
Conclusion: This study showed that there was a fairly Adherence in the use of Inotropes based on AHA/ACC guideline. Furthermore, large studies are recommended to confirm the results of present study with long term duration.
