ارزیابی الگوی مصرف و تجویز آلبومین در بیماران بستری در بیمارستان سینا و میزان تطابق آن با الگوی استاندارد بین المللی به مدت 6 ماه

Abstract

مقدمه مطالعات ارزیابی استفاده از داروها یا DUE (Drug Utilization Evaluation)، با هدف توصیف، بررسی و در نهایت بهبود کیفیت استفاده از داروها انجام می گیرند. آلبومین داروی بیولوژیك و گران قیمتی است كه به میزان زیادی نادرست مصرف میشود. با توجه به محدود بودن تولید، هزینههای زیاد و برخی مشکلات دیگر بررسی مصرف آلبومین اهمیت زیادی دارد.هدف این مطالعه با هدف بررسی میزان تطابق مصرف آلبومین در بیمارستان سینا شهر تبریز با پروتكل استاندارد انجام شد.روش کار و مواد برای ارزیابی اندیکاسیون های تجویز آلبومین و بررسی صحت تجویز آن، و محاسبه هزینه صرفه جویی شده، اطلاعات کامل تک تک بیماران جمع آوری شد و توسط داروساز متخصص و مجرب بالینی مورد بررسی قرار گرفت. بررسی صحت اندیکاسیون، براساس چک لیست وزارت بهداشت ایران و گایدلاین های استاندارد انجام شد. همچنین در صورت مواجهه با موارد مصرف نادر و ذکر نشده در گایدلاین ها، بررسی مطالعات مختلف در زمینه آن اندیکاسیون بطور اختصاصی جهت تعیین صحت اندیکاسیون انجام گرفت. یافته ها در مطالعه حاضر 151 بیمار که در بازه زمانی یک ساله در بیمارستان سینا تبریز آلبومین دریافت کردند، مورد بررسی قرار گرفتند 85 نفر (2/56 درصد) را مردان تشکیل می دادند. بیشتر بیماران در بخش مراقبت های ویژه بستری بودند. بیشترین میزان مصرف آلبومین در بخش مراقبت های ویژه عفونی بود. بیشترین دلیل تجویز نادرست در این بخش-ها مربوط به هایپوآلبومینمی بود. بیشترین درصد تجویز نادرست نسبت به کل تجویز انجام شده در بیمارستان مربوط به متخصصین عفونی بود.نتیجه گیری حدود نیمی از تجویزهای انجام شده در بیمارستان سینا فاقد اندیکاسیون لازم بودند. بیشترین موارد تجویز در این بیمارستان در بخش عفونی بود. بیشترین مصرف نادرست با اندیکاسیون هایپوآلبومینمی بود., Introduction Drug Utilization Evaluation (DUE) studies are intended to describe, review, and ultimately improve the quality of the drug usages. Albumin is an expensive biological drug that is widely consumed inappropriately. Due to limited production and high costs, evaluating albumin usage is important.Aim This study aimed to evaluate the compatibility of albumin administration with standard practical protocol in Sina Hospital affiliated to Tabriz University of Medical Sciences.Materials and Method To evaluate the indication-based administration of albumin and determine the cost saved by preventing inappropriate use, complete clinical, paraclinical and demographic information for each patient were collected and examined by an expert specialized clinical pharmacist. Appropriateness of the indications was evaluated based on the Iranian Ministry of Health checklist and the standard practical guidelines. Also, in the case of unexplained or crude indications that not considered in the guide lines, various studies were reviewed in the field to determine the accuracy of the indication.Results In the present study, 151 patients received albumin during a one-year period. 56.2% were men. The most of the patients was admitted in the ICU. The highest number of albumin vials was used in the Infectious intensive care unit. Most common case of inappropriate use was due to hypoalbuminemia. The most inappropriate use of albumin among various specialties was performed by infectious diseases specialists. Conclusion approximately one half of albumin prescriptions at Sina hospital were unnecessary. The majority of patients received albumin inappropriately was admitted to infectious ward. The most common case of inappropriate use was due to hypoalbuminemia.
