بررسی علل تجویز و مصرف غیر استاندارد دارو در کشور و راهکارهای بهبود آن از دیدگاه دانشجویان و اساتید دانشکده داروسازی تبریز در سال 1394

Abstract

مقدمه مصرف غیرمنطقی داروها پیامدهای ناگواری درپی دارد. در این میان پزشکان و داروسازان نقش حساسی در ایجاد و اشاعه مصرف غیرمنطقی داروها، جهت‌ گیری و هدایت فرهنگ دارو درمانی به سوی روش‌های صحیح تجویز، تهیه و مصرف داروها را دارند. هدف این مطالعه بررسی علل تجویز و مصرف غیراستاندارد دارو در کشور و راهکارهای بهبود آن از دیدگاه دانشجویان و اساتید دانشکده داروسازی تبریز در سال 1394 بود.روش‌کار این مطالعه مقطعی از نوع توصیفی-تحلیلی بود. برای جمع‌آوری داده‌ها پرسشنامه‌ای در 6 حیطه طراحی و توسط دانشجویان دکتری عمومی داروسازی و اساتید دانشکده داروسازی دانشگاه علوم پزشکی تبریز تکمیل گردید. برای تعیین روایی پرسشنامه نظرات چندین نفر از اساتید صاحب نظر منظور گردید و برای پایایی پرسشنامه از روش الفای کرونباخ که مقدار عددی آن 832/0 بود استفاده شد. یافته‌ها مهمترین علل تجویز و مصرف غیر منطقی دارو از نظر اساتید، نظارت ناکافی معاونت‌های درمان و سازمان نظام پزشکی به عملکرد پزشکان (75/0±45/3 امتیاز)، آموزش ناکافی به مردم در مورد الگوی صحیح مصرف دارو در رسانه‌های گروهی (93/0±35/3 امتیاز)، ارزان بودن قیمت دارو (91/0±25/3 امتیاز) و از نظر دانشجویان تمایل بیماران به خود درمانی (63/0±49/3 امتیاز)، پزشک سالاری و عدم تعامل کافی پزشکان با داروسازان و سایر همکاران گروه پزشکی (71/0±41/3 امتیاز)، سواد و سطح آگاهی پایین بیماران در مورد خطرات مصرف نابجای دارو (62/0±39/3 امتیاز) بودند، از این منظر تفاوت میان نظرات دانشجویان و اساتید وجود دارد. از‌طرفی میانگین امتیازات هر حیطه میان اساتید و دانشجویان تفاوت معنی¬داری نداشت.نتیجه‌گیری عدم نظارت کافی از طرف ارگان‌های مدیریتی و نظارتی، عدم آگاهی جامعه از مصرف صحیح داروها و تمایل بیماران به خود درمانی و عدم تعامل پزشکان با سایر همکاران حوزه بهداشت اصلیترین علل مصرف غیر استاندارد دارو توسط بیماران میباشند، مهم¬ترین راهکارهای پیشنهادی شامل تشدید نظارت سازمان¬های مربوطه بر عملکرد گروه پزشکی و همچنین افزایش سطح آگاهی عمومی در مورد مصرف استاندارد دارو و نیز برطرف نموده مشکلات اقتصادی داروخانه و داروسازان جهت کاهش مصرف غیراستاندارد دارو در کشور می¬باشد, Introduction Irrational drug consumption lead to unpleasant consequences. Physicians and pharmacists play an important role in either expanding this irrational use, and follow the proper patterns of drug prescription and consumption.Aim The aim of this study was to causes of non-standard drug prescription and consumption in Iran, and solutions of this ivestigate, from the view of pharmacy students and professors of Tabriz University of medical sciences in 2015.Methods This cross-sectional study descriptive and analytical and using a census method in six field’s questionnaire evaluated the pharmacy students and professors of pharmacy faculty of Tabriz University of medical sciences. The validity of the questionnaire was confirmed by experts and the reliability was confirmed by Cronbach’s Alpha method with the numerical value of 0.832. Results The most important causes of irrational use of prescribed medicines in terms of teachers, was insufficient surveillance of the health and treatment deputy on physicians’ prescriptions (3.45± 0.75 of the total sum of 4 points), insufficient educational programs of the media on the issue of proper patterns of medicine use (3.35± 0.93) and low drug prices (3.25±0.91) and from the perspective of the pharmacists students; patients’ tendency for use of medicine without physicion order arbitrary treatment (3.49±0.63), physician based health system and lack of physician-pharmacist colaboration (3.41± 0.71) and illiteracy and low education and knowledge of consequences of inappropriate drug use, by patients (3.39±0.62) are the most prominent causes of irrational drug prescription and consumption. From this perspective, the difference between the students and teachers there. And the average score for each area was no significant difference between teachers and students.Conclusion lack of awareness form proper use of drugs willingess of patients for Self-medication, and the lack of interactive between physician and other health workers are the main causes of non-standard use of drugs by patients, Suggested measures include intensified monitoring of the most important organizations on the performance of the health care team as well as increased public awareness about the standard drug consumption and also eliminate the economic problems pharmacies and pharmacists to reduce the consumption of substandard drugs in the my country.
