خطاهای تجویز تغذیه روده ای و تداخلات دارویی در بیماران بستری در بخش مراقبت های ویژه

Abstract

مقدمه بروز تداخلات دارویی درICU امری رایج است و نیاز به مطالعه و دقت دارد. اکثر بیماران تحت مراقبت که تغذیه روده ای برای آنان تجویز می گردد داروهای خود را نیز از همین طریق دریافت میکنند. اگر گاواژ دارو و غذا صحیح انجام نشود، عوارض مضاعفی به بیمار تحمیل خواهدشد.هدف بررسی خطاهای احتمالی تجویز دارو از راه لوله های تغذیه و تداخلات دارو-دارو در بیماران بستری در ICU بیمارستان های شهدا و امام رضاروش کار بیمارانی که غذا و دارو را از طریق لوله تغذیه دریافت می کردند وارد مطالعه شدند. اطلاعات لازم جهت بررسی نحوه انجام گاواژ و تداخلات دارو-دارو ثبت گردید و بر اساس گایدلاین ASPEN و نرم افزار Lexicomp مورد ارزیابی قرارگرفت.نتایج 208 بیمار وارد مطالعه شدند. میانگین سنی بیماران6/21± 3/59 بود. 7/44%(93بیمار) بیماران زن و3/55%(115 بیمار) مرد بودند. میانگین تعداد داروهای هربیمار 6/5 ± 2/16قلم، داروهای خوراکی2/4 ± 6/5 قلم و داروهای تزریقی2/3 ± 1/9 قلم بود. از بین 165 مشاهده در 23مورد فلاش آب و در 63 مورد زاویه حداقل30درجه ای تخت رعایت نشد و 7 مورد داروهایی که نباید خرد شوند از طریق لوله تغذیه برای بیمار تجویز شدند. 3224 مورد تداخل توسط Lexi Comp ثبت گردید که بعد از بررسی داروساز به 964 مورد کاهش یافت. میانگین تعداد تداخلات دارویی به ازاء هر بیمار قبل بررسی توسط داروساز بالینی 3/13±5/15، پس از آن 3/4±6/4 عدد بود.نتیجه گیریحضور داروساز در تیم پزشکی، با توجه به شناختی که از اشکال دارویی موجود، ویژگی های فیزیکوشیمیایی داروها و تداخلات دارو-غذا دارد، می تواند باعث کاهش خطاهای رخ داده حین تغذیه انترال گردد. داروساز بالینی در ICUبا آموزش و تخصص منحصر بفرد خود در تمام جنبه های تجویز و تحویل دارو و واقف بودن به نتایج دقیق و شدت تداخلات دارو-دارو می تواند مانع وقوع آن شود., IntroductionThe incidence of drug interactions is common in ICU and requires studying and consideration. The majority of patients that receive enteral nutrition receive their medication in the same way. If food and medicine gavage procedure does'nt be correct, the patients will have additional complications.Aim Study of possible medication errors through feeding tubes and drug-drug interactions in ICU patients of Shohada and Imam Reza hospitalMethod Patients who received food and medicine through a feeding tube were enrolled in the study. Necessary information were recorded to evaluate drug-drug interactions and gavage procedure and evaluated by the references ASPEN and Lexicomp.Results 208 patients were enrolled. The average age of patients was 59.3. 44.7% were women and 55.3% were men. The average number of drugs per patient was 16.2 which 5.6 of them included oral (PO), and 9.1 of them, included injectable drugs. Of the 165 observations gavage procedure in 23 cases flash water and in 63 cases the bed angle was not regarded and in 7 cases drugs that should not be crashed administered through nutritional tube for patient. 3224 interactions were recorded by Lexicomp that after a review of the pharmacist the number reduced to 964. The average number of drug interactions per patient before clinical pharmacist's review was 15.5, then it became 4.6.Conclusion The presence of the pharmacist in the medical team can reduce the errors occurrence during the enteral nutrition due to his knowledge of the pharmaceutical form, the physicochemical characteristics of the drug and drug-food interactions. Clinical pharmacist in the intensive care unit can prevent interactions by his unique expertise in all aspects of the administration and delivery of the drug and recognizing the exact results and severity of drug-drug interactions.
