بررسی سطح آنزیم قلبی تروپونینI در بیماران تحت آرتروپلاستی توتال هیپ و زانو در بیمارستان شهدا تبریز در سال 1396

Abstract

مطالعه حاضر با هدف بررسی سطح آنزیم قلبی تروپونینI بعد از عمل در بیماران تحت آرتروپلاستی توتال هیپ و زانو, بیمارستان شهداء تبریزانجام گرفت.مطالعه حاضردر بازه زمانی 8 ماهه با روش نمونه گیری سرشماری و بر روی 52 بیمار کهتحت اعمال جراحی توتال هیپ و زانو قرارگرفته بودند، انجام شد. سطح آنزیم تروپونینI قبل از عمل و 48 ساعت بعد از عمل اندازه گیری و ثبت شد. الکتروکاردیوگرام بیماراننیز قبل از عمل تهیه و سپس در زمان 48 ساعت بعد از عمل تکرار و از نظر بروز تغییرات احتمالی آنالیز شد. جهت بررسی ارتباط متغیر ها از آزمون Chi Square و Pearson Correlation و Spearman استفاده شد.میانگین سنی بیماران(20 بیمار (5/38 %) مرد و 32 بیمار (5/61 %) زن) مورد بررسی 8/10 ± 9/63 سال، بود. 32 بیمار (5/61 %) جراحی توتال هیپ و 20 بیمار (5/38 %) جراحی تعویض مفصل زانو انجام دادند؛ میانگین زمان بستن تورنیکه 44/10 ± 75/92 دقیقه با بیشینه 110 دقیقه و کمینه 80 دقیقه بود.تفاوت معنی داری بین میانگین تروپونین قبل از عمل در دو گروه توتال هیپ و توتال زانو مشاهده نشد (P=0.270). اما میانگین تروپونین به طور معنی داری بعد از عمل افزایشیافته است (P<0.001). ارتباط آماری معنی داری بین سن (P=0.057)، جنسیت (P=0.900) و علائم اختصاصی قلبی مشاهده نشد. همچنین ارتباط آماری معنی داری بین مدت زمان بستن تورنیکه و ایجاد علائم قلبی مشاهده نشد (P=0.887)., The present study was conducted with thePostoperative Cardiac Troponin I Level in Patients Undergoing Total Hip and Knee Arthroplasty, Shohada Hospital, Tabrizpurpose. The present study was conducted in an 8-month period by census sampling and on 52 patients undergoing total hip and knee surgery.The level of troponin I enzyme was measured and recorded beforesurgery and after 48 hours.The electrocardiogram was prepared before surgery, and then it was repeated 48 hours after the operation and analyzed for possible changes.Chi-Square, Pearson Correlation and Spearman tests were used to examine the relationship between variables. The mean age of the patients (20 male patients (38.5%) and 32 female patients (61.5%)) was 63.9 ± 10.8 years.32 patients (61.5%) had total hip surgery and 20 patients (38.5%) had knee replacement surgery. The mean time of tourniquet closing was 92.75 ± 10.44 minutes, with a maximum of 110 minutes and a minimum of 80 minutes.There was no significant difference between the mean preoperative troponin in total hip and knee total group (P = 0.270).Mean troponin significantly increased after surgery (P<0.001). also, there was no significant relationship between age (P = 0.057), gender (P = 0.900) and specific cardiac symptoms, there was no significant correlation between duration of tourniquet closure and cardiac symptoms (p = 0.887).
