تاثیرسوچور قبل برش عضله گلوتئوس مدیوس برعملکرد ابداکتورهای هیپ در بیماران تعویض مفصل هیپ با اپروچ لترال مستقیم

Abstract

هدف از مطالعه حاضر بررسی تاثیر سوچور قبل برش عضله گلوتئوس مدیوس برعملکرد ابداکتورهای هیپ در بیماران THA با اپروچ لترال مستقیم می باشد. در این مطالعه کارآزمایی بالینی تصادفی شده، 90 بیمار کاندید THA تحت جراحی با اپروچ لترال با یک سوچور در 4 سانتی متری از تیپ تروکانتر بزرگ قبل split عضله گلوتئوس مدیوس (45=n) و بدون سوچور (45=n) قرار گرفتند. دو گروه قبل و 3 و 12 هفته بعد از عمل تحت بررسی از نظر عملکرد ابداکتور هیپ با تست ایزوکینتیک، Harris hip score و میزان falling risk با استفاده از Biodex Balance system قرار گرفتند. دو گروه از نظر سن و جنس همسان بودند. به دنبال درمان، ریسک سقوط در هر دو گروه بطور بارزی کاهش یافت که بیشترین میزان بهبودی در 3 هفته اول بود. HHS نیز در هر دو گروه بطور بارزی بهبودی پیشرونده ای داشت. امتیاز عملکردی عضله ابداکتور در سه هفته اول بعد از عمل کاهش بارزی داشته که این میزان کاهش در گروه بدون سوچور بیشتر بود، ولی در هفته 12 بعد از عمل افزایش قابل توجه و در واقع بهتر شدن امتیاز عملکرد عضله ابداکتور در هر دو روش وجود داشت. , The aim of current study is to evaluate the effect of presplitting suture of gluteus medius on hip abductor function in patients undergoing THA with direct lateral approach. In this randomized clinical trial, 90 patients undergoing THA with lateral direct approach had presplitting suture of gluteus medius in 4 cm distance from greater trochanter (n=45) or no suture (n=45). Two groups before and 3 and 12 weeks after surgery were evaluated for hip abductor muscle with isokinetic test, Harris hip score (HHS) and Fall risk with Biodex balance system. Two groups were similar in sex and age. Abductor muscle function was significantly reduced in first three weeks with higher reduction in the group with no suture, but in 12 weeks after surgery, had significant increase and in fact abductor muscle function was improved in both methods. HHS was also significantly improved in both groups. Following treatment, Fall risk in both groups were significantly reduced with higher reduction in the first three weeks.
