ارزیابی نتایج پریمتری در بیماران با گلوكوم زاویه باز

Abstract

گلوکوم بیماری آسیب عصب اپتیك و اختلال میدان بینایی میباشد و افزایش فشار داخل چشمی فقط یكی از ریسك فاكتورهای اولیه ی آن است و وجود یا عدم وجود آن نقشی در تعریف گلوكوم ندارد. پریمتری اتوماتیك استاندارد یكی از تست هایGold Standard جهت بررسی میزان درگیری میدان بینایی در این بیماری هست. جهت بررسی Reliability پریمتری در SITA از ایندکس های FN ، FP و FLاستفاده میشود. همچنین این ایندكس باعث تغییرات در Mean Deviation و Pattern Standard Deviation میشود. عوامل مختلفی از قبیل سن و جنس و قطر مردمک و شدت آسیب میدان بینایی و تکرار پریمتری بر reliability میتواند موثر باشد.هدف: در این مطالعه میزان reliability پریمتری و همچنین اثر احتمالی جنسیت و سن و قطر مردمک وشدت آسیب میدان بینایی و تجربه ی پریمتری و نسبت Cup to Disk بر روی نتایج پریمتری و reliability آنها مورد بررسی قرار گرفت.روش: پرونده بیماران باگلوکوم زاویه باز مراجعه کننده از سال 1389 تا 1393 مرکز آموزشی درمانی نیکوکاری تبریز از سال 1389 تا 1393 تحت مطالعه قرار گرفت و200 پرونده (132 مورد متعلق به بیماران مرد و 68 مورد متعلق به بیماران زن) بررسی شد. این مطالعه رتروگرید بوده و اطلاعات بیماران جمع آوری و وارد نرم افزار تحلیل آماری SPSS شد. بعلت پراکنده بودن داده ها از تست یومان-ویتنی و میانه برای مقایسه ی داده ها استفاده گردید.یافته ها: در مقایسه ی میانه FP ،FN ،FL ،MD و PSD در هر دو جنس و در گروه های سنی مختلف و یا در سایز های مختلف قطر مردمک تفاوت معنی داری دیده نشد ( 0.05p-value> ). در مقایسه ی میانه داده ها با تفکیک داشتن یا نداشتن تجربه ی قبلی پریمتری، با استفاده از آزمون یومان-ویتنی MD در گروه بدون سابقه dB 13.4- و در گروه با سابقه dB6.1- (0.000P-Value=). MD و PSD در بیماران با CDR بالا به ترتیب dB 13.14- و dB 6.22 بود و در بیماران با CDR پایین به ترتیب dB 4.53- و dB 3.88 بود ( 0.05 P-Value <). همچنین میانه 1% = FPو میانه 6% =FL و میانه 7% FN=گزارش شده است., Glaucoma is a disease with optic nerve damage and visual disorder. Increasing of intraocular pressure is one of primary risk factors for glaucoma, but not the main cause. Automated Standard Perimatry (SAP) is gold standard test for visual field assessment. FN, FP and FL are the indexes that be used for assessment of reliability of perimetry in SITA. Also these indexes can influence results of MD and PSD. Some factors such as sex, age, pupil diameter and severity of visual field damage and repeating of perimetry, can influence the result of perimetry and reliability of it.Aim: Assessment of reliability of perimetries and studying about probably effect of sex, age, pupil diameter, visual field damage and test repeating on results of perimetry and reliability of it.Method: We studied 200 cases with open angle glaucoma in medical education center of Nikookari, since 2011 to 2015. This was a retrograde study. We extracted the data and analyzed them with SPSS(statistical package for the social science) and because of scattering of data we used Mann-Whitney U test type and median for comparing of groups.Result: By comparing of medians of FP, FN, FL, MD and PSD in groups of different age or sex or pupil diameter there was no significant difference (P-Value>0.05). There was significant difference for MD by grouping with having experiences of perimetry or not. In patient without any experiences of perimetry MD was -13.4dB and in patient with previous experience of perimetry MD was -6.1dB (P-Value=0.000). Also MD in patient with better visual acuity was -5.8dB and in patient with poor visual acuity was -15.39dB (P-Value=0.000). MD and PSD with high Cup to Disc Ratio (CDR) was -13.14dB and 6.22dB, but in patient with low CDR was -4.53dB and 3.88(P-Value<0.05). Median of FP was 1% and FN=7% and FL=6%.
