بررسی ارزش پیش گویی کننده بیومارکرNeutrophil lymphocyte ratio(NLR) و بیومارکرشمارش تام نوتروفیل در مورتالیتی بیماران مبتلا به ترومبو آمبولی ریوی با ریسک پایین

Abstract

Pulmonary Thromboembolism(PTE) یکی از مشکلات اساسی حیطه پزشکی می باشد.Neutrophil to Lymphocyte Ratio(NLR)به عنوان یک بیومارکر جدید در ریسک فاکتورهای Cardiovascular diseaseو همچنین در بر آورد پروگنوز بیماریهای کاردیو واسکولار مورد استفاده قرار میگیرد که بسیار دقیقتر از شمارش لکوسیت به تنهایی میباشد.در این مطالعه سعی شده است که به ارزیابی نحوه ارتباط NLRو شمارش تام نوتروفیل در بیمارانLow risk PTE پرداخته شود.مواد و روش کار:این مطالعه بر روی کلیه بیمارانی که با تشخیص Low risk PTE در بخش CCU یا داخلی قلب بیمارستان شهید مدنیانجام گردید.داده های لازم بر اساس اطلاعات موجود در پرونده ی بیماران و شامل جنس ، سن ، مدت زمان بستری بیمارستانی ، تعداد تام گلبول های سفید ، نوتروفیل و لنفوسیت و نسبت نوتروفیل به لنفوسیت می باشد.یافته ها :از 84 بیمار مورد بررسی 7/41% مونث و 3/58% مذکر بودند.از بین بیماران 5/9% مورتالیتی و 5/90% زنده وجود داشت.میانگین سنی بیماران مورد بررسی 87/1± 17/59 سال بود.میانگین شمارشWBCبیماران87/359 ± 02/10567 بود.میانگین شمار تعداد نوتروفیل بیماران 90/345± 09/7652 با بود.میانگین شمار تعداد لنفوسیت بیماران 42/110 ± 21/2019 بود.میانگین نسبت NLR د ر بیماران50/0 ± 12/5 بود.در بررسی ارتباط بین NLR با مورتالیتی بیماران Low risk PTE تفاوت معنی داری دیده شد) 043/0P= ( بطوریکه میانگین NLR در بیماران فوت شده 03/3 ± 20/9 بود در حالی که میانگین NLR در بیماران زنده43/0 ± 69/4 بود., Pulmonary thromboembolism(PTE) is one of life threatening disease in medicine.now days Neutrophil to Lymphocyte ratio(NLR) is a new biomarker that is used as a risk factor in Cardiovascular disease and also evaluating prognosis of cardiovascular disease. This biomarker is more accurate than WBC count,so the aim of the present study was to explore the prognostic values of NLR and Neutrophil count in patients with Low risk PTE.Material & Methods:This study was carried out of patients diagnosed with Low risk PTE admitted in the madani Hospital.Datas are all collected based on information contained in the records of patients including age, sex,duration of admission,WBC & Neutrophil & Lymphocye count and NLR.Results:Eighty four patients met our selection critera.41.7% were female and 58.3% were male.9.5% of patients were dead and 90.5% were alive.The average age of patients were 59.17±1.87 years.The average of WBC count were 10567.02±359.87.The average of Neutrophil count were 7652.09±345.90 and the average of Lymphocyte count were 2019.21±110.42.the average of NLR were 5.12±0.50.There was a significant difference between NLR and mortality of patients with Low risk PTE(P-value=0.043) so that the average of NLR in died patients were 9.20±3.03 but in alive patients were 4.69±0.43.
