میزان وعلل خطاهای پزشكی مربوط به بیهوشی در جراحی های ارتوپدیك در بیمارستان شهدا تبریزسال 94

Abstract

هدف از انجام مطالعه حاضر؛بررسیمیزان و علل خطاهای پزشکی مربوط به بیهوشی در جراحی های ارتوپدیک در سال 1394 در مرکز آموزشی درمانی بیمارستان شهداء دانشگاه علوم پزشکی تبریزمیباشد.مواد و روشهادر یک مطالعه توصیفی مقطعی،تمامی خطاهای بیهوشی صورت گرفته در طی سال 1394 در جراحیهای ارتوپدی مرکز آموزشی درمانی بیمارستان شهدا دانشگاه علوم پزشکی تبریز مورد بررسی قرار گرفت. نوع عمل جراحی، داده های دموگرافیک بیماران، نوع خطا،فرد مسئول بروز خطا، علت بروز خطا، و نتیجه نهایی خطا، مورد مطالعه قرار گرفتند. نتایجدر طول سال 1394، 30 مورد خطای مربوط به بیهوشی در بیمارستان شهدا دانشگاه علوم پزشکی تبریز گزارش شد. در مجموع 30 خطای مشاهده شده؛ 20 خطای (66.7%) دارویی، 8 خطای (26.6%) تکنیکی، شایعترین انواع خطا بودند. عدم دقت و تمرکز کافی در 14 مورد (46.7%)، و عدم هماهنگی مناسب بین همکاران در 13 مورد (43.3%)؛ به ترتیب شایعترین علل بروز خطا بودند. در مطالعه حاضر، عوارض جدی ناشی از بروز خطا مشاهده نشد. , The aim of present study is to evaluate of rate and causes of the anesthesia related medical errors in patients undergoing orthopedic surgery during year 2016 in Shohada Educational Hospital of TabrizUniversity of Medical Sciences.Methods & Materials:In a cross sectional descriptive study, all anesthesia related medical errors in orthopedic surgery during year 2016 in Shohada Educational Hospital of TabrizUniversity of Medical Scienceswere evaluated. Type of surgery, demographic information of patients, type of error, person responsible for error,cause of error, and final results of error, were studied. Results:During year 2016, 30 errors relatedanesthesia were reported in Shohada Educational Hospital of TabrizUniversity of Medical Sciences. Among all 30 errors, 20 (66.7%) medication errors, and 8 (26.6%) technical errors were the most common types of errors. Lack of precision and focusing enough in 14 cases (46.7%), and insufficient coordination between colleagues in 13 cases (43.3%) were the most common causes of errors respectively. In present study, serious complications due to errors were not seen.
