بررسی میزان تبعیت از درمان و عوامل موثر برآن در بیماران مبتلا به روماتیسم پالندرومیك مراحعه كننده به درمانگاههای فوق تخصصی روماتولوژی شهر تبریز از سال 84تا93

Abstract

روماتیسم پالیندرمیک با حملات مکرر و نامنظم بدون تب آرتریت و پری آرتریت مشخص می شود که در فواصل نا مشخص رخ داده و به مدت چند ساعت تا چند روز طول می کشد.این بیماری در کشور ما نادر نبوده و کنترل آن به عنوان یک بیماری مزمن دارای اهمیت بالایی است.پیروی از دستورات درمانی از دیرباز به عنوان یک موضوع چالش برانگیز در حوزه سلامت مطرح بوده است.لذا هدف از این مطالعه بررسی میزان تبعیت از درمان در بیماران مبتلا به رماتیسم پالیندرمیک مراجعه کننده به درمانگاه های فوق تخصصی روماتولوژی شهر تبریز از سال 1384 تا 1393 بوده است.مواد و روش ها: در این مطالعه توصیفی-تحلیلی 61 بیمار از 100بیمار مبتلا به پالیندرمیک روماتیسم مراجعه کننده به درمانگاه های فوق تخصصی روماتولوژی دانشگاه علوم پزشکی تبریزکه شرایط ورود به مطالعه را داشتند بررسی شدند.جهت تعیین مشخصات پایه و دموگرافیک و رضایت از درمان از چک لیست ارزیابی کمپلیانس درمانی بیماران روماتولوژیک استفاده شد.سپس بیماران براساس پرسشنامه CQR5 به دو گروه کمپلیانت و غیرکمپلیانت تقسیم شدند.برای تجزیه و تحلیل داده ها از نرم افزار تحلیلی آماری SPSS Version 22 استفاده شد.نتایج: 65.6% بیماران کمپلیانت و 34.4%غیرکمپلیانت بودند. بین میزان رضایت از درمان و تبعیت از درمان رابطه معنا دار وجود داشت.همچنین میزان تبعیت از درمان در بیماران ساکن شهر نسبت به بیماران ساکن روستا و بیماران ساکن تبریز نسبت به شهرهای دیگر به طور معنا داری بیشتر بود. داشتن باور غلط ، احساس بهبودی و عدم اعتماد به دارو ،ترس از عوارض دارو،فراموشی و نهایتا عارضه دارویی به ترتیب شایع ترین علل عدم کمپلیانس بیماران بودند, Palindromic rheumatism consists of episodic afebrile arthritis and periarthritis occurring unpredictably and lasting for hours to days.This disease is not rare in Iran according to previous studies.Adherence to therapy and therapeutic compliance in controlling chronic disease has always been a big challenge for the healthcare system and since palindromic rheumatism is a chronic diease then long term control is of a great importance.The aim of this study was to determine the compliance to treatment and factors associated with it in palindromic rheumatism patients referred to rheumatology clinics of Tabriz university of medical sciences.Research Method: In a descriptive-analytical study 61 patients out of 100 patients meeting the inclusion criteria were entered the study.Primary and demographic information of patients were registered in a checklist for assessing the compliance to treatment in patients with rheumatologic disease and then they were divided to groups of compliant or non-compliant on the basis of a 5 item Compliance Questionnare for Rheumatology (CQR5) .SPSS version 22 was used for analysis of the data.Results: 65.6% of patients were compliant and 34.4 were non-compliant.Statistically there is a positive relationship between patients’ satisfaction of therapy and compliance to treatment.There also is a positive relation between being urban and being compliant.False beliefs about medications,feeling better and distrusting the medication,fearing the side effects of medications,forgetting the medications and finally side effects are respectively the most common causes of non-compliance according to this study.Concusion: Despite its importance and prevalence unfortunately there is no other study being done around compliance to treatment of patients with palindromic rheumatism.So we recommend that more studies in this field are needed for further information.
