بررسی اثر پیوند کلیه بر سطح سرمی استئوپروتگرین و سفتی عروقی بیماران دیالیزی

Abstract

استخوان یک بافت پیچیده است که متشکل از سلول¬ها، ماتریکس کلاژنی و اجزای غیرآلی می¬باشد. در بیماران مبتلا به نارسایی کلیه، انواع مختلفی از بیماری¬های متابولیک استخوان بروز می¬کند که ناشی از اختلالات پاراتیروئیدی و متابولیسم ویتامین D هستند. هدف از این مطالعه بررسی اثر پیوند کلیه بر سطح سرمی استئوپروتگرین سفتی عروقی قبل و بعد از پیوند کلیه بود.مواد و روشها:در یک مطالعه مقطعی توصیفی- تحلیلی که در دپارتمان نفرولوژی دانشگاه علوم پزشکی تبریز بر روی بیماران تحت پیوند کلیه انجام دادیم، اثر پیوند کلیه بر سطح سرمی استئوپروتگرین سفتی عروقی قبل و بعد از پیوند کلیه را مورد بررسی قرار دادیم.نتایج:19 بیمار مرد و 11 بیمار زن بودند. میانگین سن بیماران مرد و زن به ترتیب 21/16 ± 84/37 سال و 04/12 ± 36/38 سال بود (927/0=P). میانگین میزان ABI اولیه در بیماران 24/0 ± 28/1 بود. میانگین میزان ABI شش ماه بعد در بیماران 16/0 ± 07/1 بود. کاهش معنی دار در میزان ABI شش ماه بعد نسبت به میزان ABI اولیه در بیماران مورد مطالعه وجود داشت(001/0P<). میانگین سطح OPG اولیه در بیماران 13/329 ± 49/621 بود، میانگین سطح OPG یک هفته بعد از پیوند در بیماران 10/296 ± 24/549 بود. میانگین سطح OPG شش ماه بعد از پیوند در بیماران 54/210 ± 66/466 بود. , Bone is a complicated tissue composed of cells, collagenous matrix, and inorganic elements. In patients with renal failure, different types of metabolic bone diseases develop as a result of parathyroid disorders and vitamin D metabolism. The aim of this study was to evaluation the effect of kidney transplantation on serum level of Osteoprotegrin and arterial stiffness before and after kidney transplantation.Methods and Materials:In a cross-sectional and descriptive-analytical study, which was conducted in Nephrology department of Tabriz University of Medical Sciences on patients with kidney transplantation, the effect of kidney transplantation on serum level of Osteoprotegrin and arterial stiffness before and after kidney transplantation was evaluated.Results:19 patients were male and 11 patients were female. The mean age of male and female patients was 37.84 12.04 years and 38.36 12.04 years, respectively (P=0.927). The mean initial ABI of patients was 1.28 0.24. The mean ABI of patients after six months was 1.07 0.16. A significant decrease was observed in the ABI level of patients as compared to the initial ABI level six months later (P<0.001). The mean initial OPG of patients was 621.49 329.13, and the mean OPG level one week after kidney transplantation was 549.24 296.10. The mean OPG level six months after transplantation was 466.66 210.54.
