بررسی و مقایسه تعیین گرهای ترخیص با رضایت شخصی قبل و بعد از اجرای طرح تحول نظام سلامت در بیمارستان امام رضا(ع) تبریز در سال 94

Abstract

ترخیص با رضایت شخصی یکی از مشکلات شایع در بیمارستانها بوده و برای نظام سلامت پیامدهای متعددی دارد.این مطالعه با هدف تعیین و مقایسه ی تعیین گرهای ترخیص با رضایت شخصی قبل و بعد از اجرای طرح تحول طراحی شده است.مواد و روش کار:در این مطالعه توصیفی تحلیلی تعداد 200 بیماراز مجموع 1494 بیمار ترخیص شده با رضایت شخصی از بیمارستان در سال 94 بامطالعه قبلی در سال 91 مورد مقایسه قرار گرفته است.نمونه ها به صورت تصادفی انتخاب و داده ها با استفاده از پرسشنامه معتبر و به روش مصاحبه جمع آوری شده و با نرم افزاز spss تحلیل شدند.یافته ها:در این مطالعه، 3.9% با رضایت شخصی مرخص شده اند، در حالیکه در سال 91 این میزان برابر 5.49% بود. 15% بیماران فوق بعد از ترخیص مجددا بستری شدند که در مطالعه 91این میزان برابر 13.5% بود.در هر 2 مطالعه، میانگین رضایت بیماران از بیمارستان (بین 0 تا 20) تقریبا برابر با 13 بود. با این حال رضایت از عوامل انسانی و محیطی درون بیمارستان در مطالعه ی حاضر به شکل معنی دار آماری بیشتر شده بود., Discharge against medical advice is one of the common problems in hospitals and it has many complications for health care system. This study was conducted to determine and compare the determinants of DAMA before and after health system evolution project.Methods: in this descriptive and analytical study,200 patients out of 1494 discharged patients against medical advice in the year 1394(2015-2016)were entered the study and then were compared to the former similar study .patients conducted in the year 1391(2012-2013).the samples were chosen randomly using a valid questionnaire and interview and all the data were analysed by spss software.Results: In this study, 3.9% of patients were DAMA, while in the previous study it was 5.49%. in our study, 15% of patients were re-admitted within the period of study, while in the last study it equaled 13.5%. In both of the studies the mean of patients’ satisfaction from hospital was 13 (from 0 to 20). Anyhow, the satisfaction from staff and environmental factors was higher in recent study.
