بررسی سهولت تهویه توسط ماسک در سه تکنیک E-O, E-C, Thenar Eminence

Abstract

تهویه با بگ و ماسک یک جزء اصلی در برقراری راه هوایی و القا بیهوشی عمومی است. اگرچه تهویه به وسیله بگ و ماسک در ظاهر ساده است, اما انجام صحیح آن می تواند مشکل باشد. تهویه به وسیله بگ و ماسک باید توسط فرد مجرب انجام شود تا قادر باشد آن را محکم و مناسب در جای خود نگه داشته و تهویه بافشار مثبت لازم انجام شود تا جایی که عدم وجود تجربه در تهویه به وسیله بگ و ماسک یک کنتراندیکاسیون نسبی برای انجام آن است. اهمیت تهویه با بگ و ماسک و ابداع روش های جدید یکی از موضوعات مهم در بحث اداره راه هوایی می باشد. ما در این تحقیق بر آنیم که سهولت تهویه با یک تکنیک جدید تهویه دو دستی با بگ و ماسک را که در طی آن فرد احیاگر با دو دست ماسک را به صورت E-O نگه می دارد. با دو روش تهویه مرسوم دیگر E-C techniqual و Thena Eminence در دو گروه مبتدی و ماهر بر روی مانکن مورد بررسی قرار دهید. و با استفاده از نتایج به دست آمده از این تحقیق، سالارین روش تهیه با بگ و ماسک را بین روش های مذکور برای تهویه با بگ و ماسک توسط افراد مبتدی معرفی نماییم. مواد و روش ها:این مطالعه بر روی مانکن skill lab دانشکده پزشکی انجام شد.200 داوطلب شامل 2 گروه 100نفره به شرح زیر وارد مطالعه شدند. گروه یک: 100 نفر که مسلط به تهویه با بگ و ماسک بودند. و شامل رزیدنتهای سال بالایداوطلب اورژانس یا بیهوشی و هیئت علمی های این دو گروه که مسلط به تهویه با بگ و ماسک بوده و بیش از 3 سال سابقه کار داشتند. گروه 2: 100 نفر از اینترن های گروه طب اورژانس و یا رزیدنتهای سال یک گروه طب اورژانس یا بیهوشی بودند که نحوه ی انجام تهویه با بگو ماسک را ملاحظه کرده بودند ولی تجربه ی این مهارت را نداشتند. تهویه با الگو بگ: 3 روش توسط داوطلبین انجام می شد و 2 استاد مسئول نمره دهی به داوطلبین بودند. اگر فرد آزمون دهنده از هر دو استاد نمره 4 می گرفت در دسته یideal قرار می گرفت. اگر از هر دو استاد نمره 3 یا 4-3 یا 3-4 می گرفت در دسته یaverage قرار می گرفت و یا اگر از یکی از اساتید نمره 1 و یا 2 می گرفت در دسترسی ضعیف قرار می گرفت. نهایتاً داده ها در قالب نرم افزار spss تحلیل شد و مقایسه قابلیت روش ها براساس آزمون chi – squer انجام شد و p-value<0.05 در سطح معنی دار انتخاب شد. یافته ها :در تهویه با بگ و ماسک به روش E-C و thenar eminence اختلاف معنی داری بین دو گروه مبتدی و متبحر وجود نداشت (p_values = 0.697 , 0.01) ولی در روش E-O اختلاف معنی دار بین دو گروه مبتدی و متبحر وجود داشت (p<0.0001). مبتدی ها روش E-O را بهتر انجام دادند و از نظر انجام سه روش نیز در گروه مبتدی ها اختلاف معنی دار وجود داشت و روش E-O را بهتر انجام دادند. , Bag-mask ventilation is essential work in airway management and general anesthesia .even though Bag-mask ventilation seems simple but in practice doing it correctly can be difficult. Bag-mask ventilation should be done by qualified person to be able keep it locks in place appropriate and ventilation be done with enough positive pressure as far as the lack of experience in the Bag-mask ventilation is a partial contraindication for doing it .the importance of Bag-mask ventilation with new methods of airway management is one of the most important issues in debates.at this study we want to compare the ease of new technique in Bag-mask ventilation with two others traditional method (Thenar Eminence and E-C technique).in new method practitioner shall keep the mask with two hands on patient or model face like E-O . this three way ventilation will be study in 2 groups of beginner and expert , then by using the results of this study we will recommend the easiest way for Bag-mask ventilation.Materials and methods:This study is a prospective and has been done on model at skill lab of medical faculty. 200 volunteers including 2 groups of 100 persons were enrolled in study. The first group was consist of 100 expert persons: the senior residents of anesthesiology and emergency department,the professors of anesthesiology and emergency department whom have had experience from 3 years ago. The second group was consist of 100 beginners persons: junior residents of anesthesiology and emergency department and senior student of medical science whom had studied the bag mask ventilation but they did not have enough experience.Three way of ventilation were done whit this group and 2 professors were responsible for scoring the practitioners. If the scores of both professors was 4, that was ideal .If the scores was 3,3 or 3,4 or 4,3 that was average and if the any scores was under 3 that was poor score.at the end of study data is analyzed in the SPSS software and the methods of bag mask ventilation were been compared by Chi_square test.the p_value<0.05 was significant. Results:there was no significant difference between beginners and experiences in E_C ventilation method and thenar eminence method (p_values are consecutively 0.697, 0.01) but there wassignificant difference between beginners and experiences in E_O method (p<0.0001).the beginners had better performance than the other group .there was significant difference between practice of three methods in beginner group (p<0.001) and their performance in E-O method was better than the others methods.
