مقایسه دو روش جراحی در درمان در رفتگی های حاد مفصل کوراکوکلاویکولار

Abstract

Introduction: Acromioclavicular dislocation is one of the most common shoulder sports injuries, that appropriate treatment of acromioclavicular joint dislocation is controversial. Acromioclavicular ligament (AC) and Coracoclavicular(CC) is the most important joint stabilizers. In joint dislocation the ligaments are torn therefore the repair of these ligaments is seems to play an important role increasing in the joint stabilization. The purpose of this study was to compare treatment results two operative methods of treatment of acute acromioclavicular joint dislocation with screw and absorbable coracoclavicular suture and coracoclavicular screw Methods: In the a descriptive analytical study, 40 patients with acut acromioclavicular dislocation need to operative treatment were studied. In the target group in addition to an open reduction and fixation with screws, Coracoclavicular(CC) suture with absorbable is done. Two groups were compared in terms of clinical outcome and functional ability and complications. Results: In this study, 40 patients, mean age 31.6 8.1 years, including 39 males (97.1%)and 1 female(2.6%) were studied. Comparisons of patients performance according to Constant Score were show a significant difference between the two groups. So that has been in the CC suture was 8.7 1.7 and in other group was 10.1 2.1. Also according to the Constant on CC suture was higher points score 90% excellent and 10% good, the other group rated was 70% excellent and 30% good. Coracoclavicular(CC) distance in patients undergoing CC suture was significantly reduced and was equally to the healthy side While in the cases without CC suture was not diminished significantly coracoclavicular(CC) distance. No complications were observed in the groups., در رفتگی مفصل آکرومیوکلاویکولار یکی از شایعترین آسیب های ورزشی شانه است که همچنان در درمان مناسب دررفتگی آن کنتراورسی وجود دارد. لیگامان های آکرومیوکلاویکولار(AC) و کوراکوکلاویکولار(CC) از مهمترین پایدار کننده های این مفصل هستند. در دررفتگی مفصل این لیگامان ها دچار پارگی می شوند و این لیگامان ها نقش مهمی در افزایش ثبات مفصل داشته باشد. بنابراین هدف از این مطالعه مقایسه دو روش جراحی در درمان دررفتگی های حاد مفصل کوراکوکلاویکولار: پیچ و سوچور قابل جذب کوراکوکلاویکولار با پیچ کوراکوکلاویکولارمیصباشد.روش کار: در یک مطالعه توصیفی تحلیلی 40 بیمار دارای دررفتگی حاد مفصل که نیاز به جراحی داشتند بررسی شدند. در یک گروه علاوه بر جا اندازی باز و فیکساسیون با پیچ ، سوچور کوراکوکلاویکولار با نخ قابل جذب انجام شده است. دو گروه از نظر عوارض و پیامد بالینی و عملکردی مورد مقایسه قرار گرفتند.نتایج: در این مطالعه 40 بیمار با میانگین سنی 1/86/31 سال شامل 39 مرد(1/97%) و یک مورد زن(6/2%) مورد بررسی قرار گرفته است. مقایسه امتیاز عملکردی Constant Score نشان دهنده تفاوت آماری معنی داری بین دو گروه بوده است به طوری که در هنگام سوچور 7/17/8 و در گروه مقابل 1/21/10 بوده است. همچنین براساس امتیاز Constant امتیاز عالی در موارد سوچور 90% و خوب 10 % بوده و در گروه مقابل امتیاز عالی 70% و خوب 30% بوده است. فاصله کوراکوکلاویکولار در بیماران تحت سوچور کوراکوکلاویکولار به طور معنی داری کاهش پیدا کرده بود و با سمت سالم برابر شده بود و این درحالی است که در موارد عدم سوچور کوراکوکلاویکولار فاصله کوراکوکلاویکولار کاهش معنی داری پیدا نکرده بود. هیچ عارضه ای در گروه ها مشاهده نشد
