بررسی سهولت تهویه دو دستی تهویه با بگ و ماسک در چهار تکنیک

Abstract

از آنجایی که اینکه تهویه با بگ و ماسک یک جزء اصلی در برقراری راه هوایی و القاء بیهوشی عمومی است از اینرو باید توسط فرد مجرب انجام شود تا قادر باشد آن را محکم و مناسب در جای خود نگه داشته و تهویه با فشار مثبت لازم انجام شود.هدف:بدلیل اهمیت این موضوع ما نیز در این تحقیق درصدد بودیم که ساده ترین روش تهویه با بگ و ماسک را توسط افراد مبتدی معرفی نماییم تا میزان خطا در هنگام تهویه توسط این افراد به حداقل ممکن برسد.روش کار:این مطالعه یک نوع مطالعه متقاطع تصادفی دو سو کور می باشد. ما 120 نفر داوطلب را پس از اخذ رضایت نامه کتبی و پس از توضیح اینکه انجام این طرح هیچ خطری برای سلامتی آنها نخواهد داشت در دو گروه وارد مطالعه کردیم:گروه 1: 60 نفر که مسلط به تهویه با بگ وماسک هستند وشامل دستیاران سال بالای طب اورژانس یا بیهوشی وهیئت علمی های این 2 گروه گروه 2: 60 نفر از کارورزان گروه طب اورژانس ویا دستیاران سال یک گروه طب اورژانس یا بیهوشی که تجربه عملی این مهارت را نداشته اند.به صورت خلاصه اینکه هر فرد 4 تکنیک تهویه با بگ وماسک را اجراء کرده و دوفرد ارزیاب هم که اتند بیهوشی یا طب اورژانس با سابقه بودند ، در طرف مقابل داوطلب قرار گرفته بودند وبه نحوه انجام تهویه داوطلبBLIND شده بودند وکیفیت بلند شدن قفسه سینه مولاژ را هنگام تهویه مشاهد می کردند. بلند شدن قفسه سینه بیمار بصورت Ideal ، Average و Weak امتیاز دهی شد.سپس داده های حاصل از انجام مطالعه وارد نرم افزار spssشد و مقایسه قابلیت روشها با توجه به تجربه گروه های مورد مطالعه بر اساس آزمون Chi-square انجام شد و (p-value<0.05) بعنوان سطح معنی داری در نظر گرفته شد.یافته های اصلی :از نظر انجام هریک از روشهادر بین گروه ماهر (Experienced) تفاوت معنی داری از نظر آماری وجود نداشت که نتیجه قابل قبولی به نظر می رسید ولی در میان گروه مبتدی (Novice) از نظر انجام هریک از روشها تفاوت معنی داری از نظر آماری وجود داشت و گروه مبتدی روش Thenar Eminence (Non dominant hand)-E-C (Dominant hand)را بهتر انجام دادند, Since bag mask ventilation is considered to be a vital component in managing airway and Anesthesiology and it should be performed by an experienced person so that the mask is placed firmly and appropriately and the required Positive pressure is applied sufficiently.AimHere in this study we would introduce the simplest approach toward bag mask ventilation by novices in order to minimize the error rate while the ventilation is performed bye these people.MethodologyThis is a double blind, randomised, prospective and cross-over study, after obtaining written informed consent and persuading all volenteers that performing this plan has no risk to their health, a total of 120 volenteers were enrolled in the study that consisted of 60 experienced individuals in the field of bag mask ventilation including Emergency medicine or anaesthesia postgraduales and the faculty members of the two groups with more than 3 years of experience in bag mask ventilation and 60 emergency medicine or anaesthesia novices who have observed bag mask ventilation but had no practical experience in it.To sum up, every individual performed the four techniques of bag mask ventilation- E-C، Thenar Eminence، Thenar Eminence (Dominant hand)-E-C(Non dominant hand), Thenar Eminence (Non dominant hand)-E-C (Dominant hand)and two of the investigators who were Emergency medicine or anaesthesia attendant were blinded to the mask holding sequence positioned on either side of the volunteer to observe the quality of chest rise while ventilating (two tidal breaths of one second.)Getting chest as Ideal, Average and Weak scored.The data was recorded and analyzed using SPSS, and Chi-square test was applied to compare the efficiency of the techniques with respect to the experience of the groups involved in the study. P_Value<0.05 was considered to be statistically significant.ResultsIn performing each of the techniques among the experienced group, there was no statistically significant difference that the result considered to be acceptable, but in performing each of the techniques among the novices performed the Thenar Eminence (Non dominant hand)-E-C (Dominant hand) technique more effectively
