ارتباط سطح سرمی ناتریورتیک پپتید تایپ (BNP)Bبا میزان شانت چپ به راست(QP/QS)

Abstract

زمینه:پیتیدهای ناتری یورتیك شاملBNP، ANP و NT Pro BNPدرپاسخ به افزایش کشش دیواره بطن ها و دهلیز ها ترشح می شوند. بیماریهای مادرزادی قلبی باشانت چپ به راست مانندVSD، PDA و ASD، براساس میزان وشدت شانت،تحت درمان طبی و یاعمل جراحی قرارمی گیرند. هدف این مطالعه این است که آیا می توان براساس سطح سرمیBNP،شدت بیماری راتشخیص واقدامات درمانی و یاجراحی راانجام دادوازموربیدیتی وبروزمکررعفونتهای ریوی ونارسایی شدیدقلبی،اختلال دررشدومورتالیتی ا ین بیماریهاازیکطرف وتصمیم عجولانه برای مداخلات جراحی زودرس جلوگیری کرد.مواد و روشکار:در این مطالعه توصیفی – تحلیلی، 30 مورد از شیرخواران وکودکانی که به علت سوفل قلبی،دیسترس تنفسی،اختلال دررشد(FTT)،پنومونی های مکرر،کاردیومگالی درعکس قفسه سینه(CXR)دربخش قلب کودکان یاICUکودکان بستری گردیده وموردمشاوره قلب قرارگرفته بودند ویابه صورت سرپایی به درمانگاه قلب مراجعه نموده بودند،پس ازانجام اکوکاردیوگرافی و تشخیصPDA، ASD و VSD، واردمطالعه شده وسطح سرمیBNPدرهمان روزاندازه گیری شده است.سپس میزان شانت چپ به راست (QP/QS)براساس یافته های اکوکاردیوگرافی باسطح سرمی BNPمقایسه گردید تامشخص شودآیاارتباط معنی داری میان سطح سرمیBNPوشدت شانت براسا س یافته های اکوکاردیوگراف ی ومیزان اختلال همودینامیک وجودداردیاخیر. نتایج:از 30 بیمار بررسی شده، 13 بیمار (3/43%) مذکر و 17 بیمار (56/7%) مونث بودند. میانگین سطح سرمی BNP در ASD=281/8و VSD = 208/5 و PDA = 74/6 بوده است. میانگین سطح سرمی BNP و اندازه شنت چپ به راست در بیماران مورد بررسی با P < 0/0001 ارتباط معنی داری داشت. میانگین سطح سرم BNP در تمامی بیماران برابر 194/9 ± 21/2 بود. ارتباطمثبتیمیانسطحسرمیBNPونسبتQP/QSدرمیانتمامبیمارانمشاهدهگردید (P = 0/018). میانگین BNP اندازه گیری شده در بیماران با QP/QS<1/5 برابر 30/83±2/4، در بیماران با QP/QS<2 1/5 برابر217/88±44/6و برایQP/QS2 برابر با272/8 ±51/8 بود. ارتباط معنی داری میان سطح سرمیBNPومیزان شانت مشاهده گردید(P = 0/006). برای تشخیص بیماران باQP/QS > 1/5حساسیت واختصاصیت 100%و 83 /3%برای سطح سرمی BNP برابر 36/95 pg/mlمشخص شد. , Natriuretic peptides including BNP, ANP and NT Pro BNP are secreted in response to increased wall tension of ventricle and atrium.Congenital heart disease with left to right shunt, such as VSD, PDA and ASD, based on the extent and severity of the shunt, are underwent medical treatment or surgery.The purpose of this study is to answer this question that is it possible to get information about the disease severity or perform surgery and medical treatment and prevent the morbidity and frequency of respiratory infections and severe heart failure, impaired growth and mortality and on the one hand to avoid hasty decisions for early surgical intervention by using serum levels of BNP.Method:In this cross - sectional study, 30cases of infants and children thatbeen hospitalizedin cardiology or ICU ward of Tabriz Children’s Hospital due to heart murmur, respiratory distress, failure to thrive (FTT), recurrent pneumonia, cardiomegaly on chest x-ray, entered in the study after the diagnosis of PDA, ASD and VSD by echocardiographyand the serum BNP levels were measured in the same day. Then the rate of left to right shunt (QP / QS) based on echocardiographic findings was compared with serum levels of BNP to determine that is there a significant correlation between serum levels of BNP and the rate of the shunt on the echocardiographic findings and the hemodynamic disorder or not.Results:Of 30 patients, 13 patients (43.3%)were males and 17 (56.7%) were female. Pro BNP serum levels had a significant correlation with the left to right shunt type(P = 0.01).The mean serum levels of Pro BNP has been in ASD=281.8VSD = 208.5andPDA = 74.6. The mean serum levels of Pro BNP had a significant correlation with the size of left to right shunt with P <0.0001. A pro BNP serum level in all patients was194.9 ± 21.2. There was a positive correlation between serum levels of BNP and QP / QS in all patients (P = 0.018).The mean serum levels of Pro BNP in patients with QP / QS <1.5was30.83 ± 2.4 and in patients with1.5 QP / QS< 2 was 217.88 ± 44.6 and for QP / QS2was 272.8 ± 51.8. There was a significant correlation between serum levels of BNP and the rate of the shunt (P = 0.006). The sensitivity and specificity for the serum levels ofBNP equal to 36.95pg / ml in diagnosing patients with QP / QS> 1.5 were 100% and 83.3% respectively.
