ارزش تشخیصی گلوکومتر در مقایسه با روش آزمایشگاهی در هیپو گلیسمی نوزادان

Abstract

Introduction : Hypoglycemia in neonates is an emergency condition requiring immediate treatment to prevent serious outcomes. Most of hypoglycemic infants are asymptomatic; hence, screening is needed for high-risk infants. Today there are many different types of glucometers. Check the accuracy of these devices has been studied in children and adults. But glucometer accuracy in the diagnosis of neonatal hypoglycemia has not been adequately investigated.. Aim : The goal of this study was to determine the value of the glucometer in diagnosis of hypoglycemia in neonates . Material and Methods : This cross-sectional study was carried out on patients who referred to neonatal wards in Al Zahra hospital of Tabriz 2012-2013. 100 infants who had risk factors of hypoglycemia were chosen and blood glucose were tested with Accu check glucometer,( under construction of America ), If the blood glucose was below 60mg/dl, simultaneous 2 ml of venous blood was drawn and was sent to the reference laboratory. In the laboratory within half an hour glucose was measured by glucose oxidase method by auto analyzer selectra 2 made in Netherlands . The value were then statistically analyzed using paired T-test and ROC curve for test Validity . Results: The sensitivity of the glucometer was 45.45 % , specificity was 74.6% , accuracy was 65 % , positive predictive value was 46.8 % and a negative predictive value was 73.5 %. There was a weak correlation between glucometer and laboratory results , and the mean blood glucose measurement with laboratory was significantly higher than glucometer .P <0.006, هدف از این مطالعه بررسی صحت گلوکومتر در تشخیص هیپوگلیسمی نوزادان در مقایسه با آزمایشگاه می باشد.مواد و روش ها :این مطالعه از نوع توصیفی مقطعی می باشد . که بر روی نوزادان ارجاع شده به بخش نوزادان بیمارستان الزهرا طی سال های 90 لغایت 91 انجام گرفت. 100 نوزادی که دارای ریسک فاکتور های هیپوگلیسمی بودند ، انتخاب و توسط گلوکومتر accu check performa ساخت آمریکا قند خون آنها چک شد، در صورتی که گلوکز خون زیرmg/dl 60 بود همزمان 2سی سی خون وریدی جهت اندازه گیری قند خون به ازمایشگاه رفرانس ارسال شد ، در آزمایشگاه قند خون در عرض نیم ساعت با روش گلوکز اکسیداز توسط دستگاه اتوانالیزر selectra 2 ساخت هلند اندازه گیری شد. سپس ارزش تشخیصی گلوکومتر توسط انالیز های آماری و با استفاده از pair t-test بدست آمد از منحنی راک نیز برا ی بدست آوردن حساسیت و ویژگی استفاده شد.یافته ها :در این مطالعه حساسیت گلوکومتر 45/45% ، ویژگی 6/74% ، صحت 65% ، ارزش اخباری مثبت 8/46% و ارزش اخباری منفی 5/73% بدست امد.همچنین بر اساس انالیز داده ها ارتباط ضعیفی بین نتایج حاصل از گلوکومتر و آزمایشگاه وجود داشت .(ضریب پیرسون : 275/0) و به طور کلی میانگین قند خون های اندازه گیری شده توسط ازمایشگاه به طور معنی داری از گلوکومتر بالاتر بود .P Value :0/006
