مقایسه نتایج درمان شکستگی های شفت فمور در بیماران بالای 60 سال با بیماران زیر 40 سال

Abstract

The purpose of this study was to determine if outcome of IMN for femoral shaft fracture in elderly patient is also acceptable. Particular challenges in this group of patients included osteopenic bone, and multiple associated medical problems frequently observed. In this cross- sectional study between 2005-2010. We reviewed the outcome of IMN for femoral shaft fracture for 72 patient and compared the results between younger than 40 years old and elderly patients. Complications and mortality analyzed for each group, then compared them between two groups with testing the Null hypothesis . the mean age at time of surgery was 46/9 (rang 20-81 years) and the mean duration of follow up was 57/3 months (rang 10-94 months). Incidence of malunion, nonunion, infection, DVT, dependence on walker and crutch, in two groups were similar and differences were not significant(p value >0/05).Incidence of mortality(P value = 0/035),knee pain and loss of motion, dependence on cane were significantly highin elderly patients (pvalue=0/007, 0/003, 0/001 respectively)., هدف این مطالعه بررسی نتایج استفاده از میله گذاری داخل مدولاری در شکستگی شفت فمور افراد مسن میصباشد . در این مطالعه مقطعی بین سالهای 1384 تا 1390، تعداد 72 شکستگی شفت فمور که تحت جاگذاری میله اینترامدولاری قرار گرفته بودند به دو گروه سنی جوانتر از 40 سال و افراد مسن تقسیم بندی شدند. عوارض، مورتالیتی در دو گروه آنالیز شده و توسط Null hypothesis مقایسه شدند.در این مطالعه نمونه گیری انجام نشد ،همه بیمارانی که معیار ورود به مطالعه را داشتند بررسی نمودیم.سن متوسط بیماران 90/46 سال ( محدوده 20-81 ) انحراف معیار 2/11 و متوسط دوره فالوآپ 57/3 ماه ( محدوده 94-10 ماه) با انحراف معیار 7/18 بود. میزان بد جوش خورده، نان یونیون ، عفونت DVT وابستگی به واکر و کراچ ، مشابه بوده و تفاوت قابل توجه نبود.(P>0/05) تفاوت میزان مرگ و میر (P=0/035)، درد زانو و دامنه حرکتی و وابستگی به عصا در افراد مسن قابل توجه بود ( به ترتیب001/0 ، 003/0 ، 007/0 P =) .
