بررسی خطاهای نوشتاری و ساختاری شکایت اصلی بیماران در شرح حال دانشجویان بخش داخلی بیمارستان امام رضا(ع)تبریز

Abstract

This study is aimed to evaluation the situation of recording chief complaint in observations of internal ward. After checking and identifying defects the solutions for correcting defects and necessary instructions is offered. Method and materials: In this study the observations of 150 externs and 150 interns and 150 residents of various wards in internal ward was selected in randomized method. And the page containing chief complaint and present illness was copied. On the basis of designed questionnaire the various properties of chief complaint of observations was checked. At the checked properties in the questionnaire for chief complaint in internal ward observations: 1-in terms of coordination between present illness and chief complaint: the most coordination was belong to residents (90%)then interns(76%) and externs(67%). 2-expressing the period of chief complaint: interns(86%)residents(80%) externs(60%) was expressed it. 3-how to write chief complaint correctly in quotation mark : the rate to observance this property in residents was 96% interns 93% and externs 80%. 4-the rate of use iatrotropic stimulus in chief complaint: residents 46%interns 33% and externs 23% were observed this point. 5-the situation of student: no one of students had commented about their observation. 6-non-use medical word in chief complaint: residents(26%) and externs(43%) had the least and most rate in order. 7-how to write referral patients correctly: externs(95%) and interns(72%) had most and least rate of accuracy in order. 8-miscellaneous issues: at the non of the observations there wasnt ugly and shameful chief complaint., این مطالعه با هدف بررسی وضعیت ثبت شکایت اصلی و اشکالات و خطاهای نوشتاری و ساختاری شکایت اصلی در پرونده های بخش داخلی طراحی شده است تا پس از بررسی و شناسایی اشکالات موجود،راهکارهای لازم جهت اصلاح نقایص موجود و آموزش های لازم پیشنهاد گردد.در این مطالعه شرح حال 150 اکسترن و 150 انترن و 150 رزیدنت بخش های مختلف گروه داخلی به طور تصادفی انتخاب شده و برگه حاوی شکایت اصلی و شرح بیماری کنونی کپی شده و بر اساس پرسشنامه طراحی شده ویژگی های مختلف شکایت اصلی پرونده ها بررسی می شود.در ویژگی های بررسی شده در پرسشنامه موجود برای شکایت اصلی ثبت شده در پرونده های بخش داخلی : 1-همخوانی وهماهنگی بین شرجپح بیماری فعلی و شکایت اصلی:بیشترین میزان در گروه رزیدنت ها با 90% سپس انترن ها با 76% و اکسترن ها با 67% کم ترین میزان بود.2-ذکر مدت شکایت اصلی: انترن ها 86%،رزیدنت ها 80% و اکسترن ها 60%. 3-طرز نوشتن صحیح "عبارتی" شکایت اصلی:رزیدنت ها 96%،انترن ها 90% و اکسترن ها 80%. 4-استفاده از محرک ایاترتروپیک: رزیدنت ها 46%،انترن ها 33% و اکسترن ها 23%. 5-موقعیت دانشجو: هیچ کدام از دانشجویان در مورد شرخ حال خود نظر نداده بودند. 6-عدم استفاده از واژه های پزشکی:رزیدنت ها با 26% کمترین میزان و اکسترن ها با 43% بیش ترین میزان. 7-نحوه نگارش درست بیماران رفرال:اکسترن ها با 85% بیش ترین و انترن ها با 72% کم ترین میزان نگارش صحیح. 8-مسایل متفرقه:در هیچ کدام ازپرونده های بررسی شده شکایات زشت و قبیح وجود نداشت.
