طرح بررسی وضعیت موجود و بهینه سازی آموزش درمانگاهی در مرکز قلب شهید مدنی تبریز

Abstract
With the changing of the outpatient management of patients, the value of inpatient education decreases, on the other hand the importance of outpatient education increasing every day. Because of the problems with the conventional outpatient clinical education programs, faculties of medicine arranging new ways to improve it and increasing patients care. This is important for general and family physician educational programs especially. From March 2009 to February 2010 in outpatient clinic of Shahid Madani Heart center and between senior and junior medical students who were in their cardiology service of education, items of patient and student satisfaction and completeness of patient files assessed by reliability and validity evaluated questionnaires in two stages: 1- Current situation with Grand stand model (GSM) and 2- Supervising model (SVM). The results compared between two models finally. In the majority of items of patient and student satisfaction questionnaires the values of satisfaction were moderate or weak. Patients files which are using as patient documents were filled incompletely and weakly. After starting SVM education, patient satisfaction with quantity and quality of student visits increased significantly (P<0.001), although time to visit was increased and patients were not satisfied with the duration of attending physician visit. Patients received complete information about their disease, work up and treatment programs. Student satisfaction increased in the items of their unauthorized visits (P<0.05). Students were unsatisfied with attending physician visits and introducing patients to them. Patients file completeness improved significantly in items of history taking and physical examination. Although in the items of writing the problem list and differential diagnosis, results were poor and were not significant., هدف از این مطالعه، طرح بررسی وضعیت موجود و بهینه سازی آموزش درمانگاهی در مرکز قلب شهید مدنی تبریز می باشد.از فروردین لغایت بهمن سال 1388 در کلینیک سرپایی مرکز قلب شهید مدنی تبریز وابسته به دانشگاه علوم پزشکی تبریز و دربین دانشجویان کارآموز و کارورز که در چرخه کاردیولوژی بودند , بر اساس پرسشنامه های از قبل تهیه شده و روایی و پایایی آنها تایید شده بود در سه موضوع رضایت بیماران، رضایت دانشجویان و تکمیل بودن پرونده های درمانگاهی در 3 مرحله 1- در وضیت موجود بروش Grand Stand Model (GSM) 2- وضعیت بعد از اصلاح بروش Report Back Model (RBM) و 3- وضعیت بعد از اصلاح بروش Supervising Model (SVM) بررسی های لازم انجام و نتایج مورد مقایسه قرار گرفتند. رضایت بیماران و دانشجویان از وضعیت موجود در حد متوسط یا پایینتر بود و پرونده های کلینیکی بعنوان اسناد پزشکی نقایص فراوان داشتند. بعد از انجام اقدامات اصلاحی بروش RBM و SVM رضایت بیماران شامل کیفیت و کمیت ویزیت توسط دانشجویان اکثرا با P<0.001 افزایش یافته بود.
