مقایسه نتایج اصلاح ژنوواروم با دو روش استئوتومی قسمت فوقانی تیبیا بصورت گوه-باز و گوه -بسته

Abstract

The aim of this study was comparison of the results of to osteotomy methods in patients with geno varus deformity. In Cohort study, 32 patients with genu varus deformity ( 42 knee) were studied. They were divided in two groups and matched according to sex and age. Patients treated with open wedge osteotomy and closed wedge osteotomy. They followed for six month after operation. And compared with together. 32 patients with genu varus deformity(10 cases(31.2%) bilateral) including 25(78.2%) women and 7(21.8%) men were studied. Incidence of complication after operation in open wedge and closed wedge osteotomy were similarly(12.5%) . Overall patients satisfaction in open wedge was 87.5% and in closed wedge was 75%. The main complication in open wedge was intraarticular fracture and in closed wedge was peroneal nerve Paralysis. There were significant difference in operation time, time of weight bearing and back to daily activities between two groups(p<0.001). In other parameters including the varus correction angle, the Lysholm and the Tegner Activity score and Insall Salvati Index were not difference., هدف از این مطالعه مقایسه نتایج روش های استئوتومی در بیماران مبتلا به دفورمیتی ژنوواروم است. در یک مطالعه همگروهی، 32 بیمار با دفورمیتی ژنوواروم(42 زانو) مورد مطالعه قرار گرفتند. آنها به دو گروه تقسیم شده و بر اساس سن و جنس همسان شدند. بیماران با استئوتومی گوه باز و گوه بسته درمان شده و برای مدت 6 ماه بعد از جراحی مورد پیگیری قرار گرفتند و با یکدیگر مقایسه شدند.32 بیمار مبتلا به دفورمیتی ژنوواروم(10 مورد(2/31%) دو طرفه) شامل 25(2/87%) زن و 7(8/21%) مرد مورد مطالعه قرار گرفتند. شیوع عوارض بعد از جراحی استئوتومی گوه باز و گوه بسته مشابه بود(5/12%). رضایتمندی کلی بیماران از روش گوه باز 5/87% و گوه بسته 75% بود. مهمترین عارضه در روش گوه باز شکستگی داخل مفصلی و در روش گوه بسته فلج عصب پرونئال بود. از نظر مدت جراحی، مدت توانایی تحمل وزن و برگشت به فعالیت روزانه بین دو گروه تفاوت آماری معنی داری(001/0>p) وجود داشت. در سایر پارامترها شامل اصلاح زاویه واروس و امتیاز Lysholm and the Tegner Activity و اندکس Insall Salvati تفاوتی وجود نداشت.
