مقایسه روش مشاهده مستقیم با پاپ اسمیر در تشخیص ضایعات پیش سرطانی دهانه رحم

Abstract

a simple, effective and low-cost alternative to cervical cytology for cervical cancer prevention is needed for low-resource settings. Materials and methods: A total of 1500 women were screened cytologically as well as clinically with direct with direct visual inspection of cervix after application of acetic acis (DVI). Women with abnormal findings in either pap smear or DVI were investigated with colposcopy and biopsies were obtaind from colposcopically assessed abnormal lesions. The gathered data were analized with observational analytic procedures and spss 15. Results: Seven women had abnormal pap smear ( 6 ASCUS-1 CIN1) with colposcopy and biopsies, 3 of 7 were actually positive. All of the 3 positive tests were positive in DVI but one of the 4 false-positive tests was positive in DVI. Nine women had abnormal DVI which colposcopy and biopsy confirmed 8 of them as premalignant or malignant lesions of cervix. Two of them were invasive cancer (negative papsmears), 3 CIN1 (one of them negative in papsmear, one CIN1, One ASCUS), 2CIN2 (one of them negative in papsmear, one ASCUS) and 1 CIN3 (negative papsmear). Discussion: Almost all studies in the developing countries have emphasized the efficacy and effectiveness of DVI., این مطالعه جهت مقایسه روش مشاهده مستقیم با پاپ اسمیر در تشخیص ضایعات پیش سرطانی سرویکس انجام گرفته است.مواد و روش ها: 1500 فرد تحت پاپ اسمیر و DVI قرار گرفتند و نتایج غیر نرمال در هر کدام با کولپوسکوپی و در صورت لزوم بیوپسی بررسی شدند. داده‌صهای به دست آمده به وسیله روشهای آماری توصیفی و با استفاده از نرم افزار آماری SPSS-I5 مورد تجزیه و تحلیل قرار گرفت.نتایج : از 1500 فرد بررسی شده، 7 نفر پاپ اسمیر غیرنرمال (6 نفر ASCUS 1 نفر CIN1) داشتند که کولپوسکوپی و بیوپسی 3 مورد آن را به عنوان ضایعه پیش بدخیم تایید کرد. 3 موردی که واقعا غیرنرمال بودند DVI مثبت نیز بودند ولی از 4 مورد مثبت کاذب فقط یک مورد DVI مثبت وجود داشت. 9 نفر DVI غیر نرمال داشتند که کولپوسکوپی و بیوپسی 8 مورد آن را به عنوان ضایعه پیش بدخیم یا بدخیم تایید کرد
